Book excerpt: Excerpt from Long-Term Care Provisions in the President's Health Care Reform Plan: Hearing Before the Special Committee on Aging, United States Senate, One Hundred Third Congress, First Session; Madison, Wi; November 12, 1993 In summary, I believe customers must be empowered to direct and manage their cares. We can no longer afford to conduct busi ness solely under the medical model. It is too demeaning and ex pensive. I personally believe customers should be able to determine how their long-term care dollars are spent. If they choose to recruit, train, and hire their own personal care attendants, that should be their choice. In fact, it may be their only choice if there are no serv ice providers in the area in which they live. About the Publisher Forgotten Books publishes hundreds of thousands of rare and classic books. Book excerpt: Many of the elements of the Affordable Care Act (ACA) went into effect in 2014, and with the establishment of many new rules and regulations, there will continue to be significant changes to the United States health care system. It is not clear what impact these changes will have on medical and public health preparedness programs around the country. Although there has been tremendous progress since 2005 and Hurricane Katrina, there is still a long way to go to ensure the health security of the Country. There is a commonly held notion that preparedness is separate and distinct from everyday operations, and that it only affects emergency departments. But time and time again, catastrophic events challenge the entire health care system, from acute care and emergency medical services down to the public health and community clinic level, and the lack of preparedness of one part of the system places preventable stress on other components. The implementation of the ACA provides the opportunity to consider how to incorporate preparedness into all aspects of the health care system. The Impacts of the Affordable Care Act on Preparedness Resources and Programs is the summary of a workshop convened by the Institute of Medicine's Forum on Medical and Public Health Preparedness for Catastrophic Events in November 2013 to discuss how changes to the health system as a result of the ACA might impact medical and public health preparedness programs across the nation. This report discusses challenges and benefits of the Affordable Care Act to disaster preparedness and response efforts around the country and considers how changes to payment and reimbursement models will present opportunities and challenges to strengthen disaster preparedness and response capacities.

This volume examines the current system of nursing home regulations, and proposes an overhaul to better provide for those confined to such facilities. It determines the need for regulations, and concludes that the present regulatory system is inadequate, stating that what is needed is not more regulation, but better regulation. This long-anticipated study provides a wealth of useful background information, in-depth study, and discussion for nursing home administrators, students, and teachers in the health care field; professionals involved in caring for the elderly; and geriatric specialists.

Book excerpt: Among the issues confronting America is long-term care for frail, older persons and others with chronic conditions and functional limitations that limit their ability to care for themselves. Improving the Quality of Long-Term Care takes a comprehensive look at the quality of care and quality of life in long-term care, including nursing homes, home health agencies, residential care facilities, family members and a variety of others. This book describes the current state of long-term care, identifying problem areas and offering recommendations for federal and state policymakers. Who uses long-term care? How have the characteristics of this population changed over time? What paths do people follow in long term care? The committee provides the latest information on these and other key questions. This book explores strengths and limitations of available data and research literature especially for settings other than nursing homes, on methods to measure, oversee, and improve the quality of long-term care. The committee makes recommendations on setting and enforcing standards of care, strengthening the caregiving workforce, reimbursement issues, and expanding the knowledge base to guide organizational and individual caregivers in improving the quality of care.

Available in PDF, EPUB and Kindle. Book excerpt: America's health care system has become too complex and costly to continue business as usual. Best Care at Lower Cost explains that inefficiencies, an overwhelming amount of data, and other economic and quality barriers hinder progress in improving health and threaten the nation's economic stability and global competitiveness. According to this report, the knowledge and tools exist to put the health system on the right course to achieve continuous improvement and better quality care at a lower cost. The costs of the system's current inefficiency underscore the urgent need for a systemwide transformation. About 30 percent of health spending in 2009-roughly $750 billion-was wasted on unnecessary services, excessive administrative costs, fraud, and other problems. Moreover, inefficiencies cause needless suffering. By one estimate, roughly 75,000 deaths might have been averted in 2005 if every state had delivered care at the quality level of the best performing state. This report states that the way health care providers currently train, practice, and learn new information cannot keep pace with the flood of research discoveries and technological advances. About 75 million Americans have more than one chronic condition, requiring coordination among multiple specialists and therapies, which can increase the potential for miscommunication, misdiagnosis, potentially conflicting interventions, and dangerous drug interactions. Best Care at Lower Cost emphasizes that a better use of data is a critical element of a continuously improving health system, such as mobile technologies and electronic health records that offer significant potential to capture and share health data better. In order for this to occur, the National Coordinator for Health Information Technology, IT developers, and standard-setting organizations should ensure that these systems are robust and interoperable. Clinicians and care organizations should fully adopt these technologies, and patients should be encouraged to use tools, such as personal health information portals, to actively engage in their care. This book is a call to action that will guide health care providers; administrators; caregivers; policy makers; health professionals; federal, state, and local government agencies; private and public health organizations; and educational institutions.

This study discovers not only a number of imaginative arguments for extending health services but also an unexpectedly wide array of passionate advocates for universalism. An innovative approach to one of the great unresolved social and political problems of our time, Health Security for All will be of interest to social scientists, health policy scholars, historians, and idealists across the political spectrum.

Available in PDF, EPUB and Kindle. Book excerpt: Excerpt from Health Care Reform: The Long-Term Care Factor: Hearing Before the Special Committee on Aging, U. S. Senate, One Hundred Third Congress, Second Session, Washington, DC, April 12, 1994 We also want to phase in a system so that we can learn from it and we can build on the best innovations at the State level. We can ensure, hopefully, as Senator Graham pointed out, that it is based more on the social model, as Opposed to a medical model, but that it blends in the best of both of those systems. That is a quick, overall summary of the President's plan. If I can, for the next few minutes, I'd like to lay out what we are doing in the Administration on Aging to complement what the President is proposing and the work that we are doing in the Department of Health and Human Services in the area of long-term care. Last year, when I came on board, we made the issue of long-term care one of our priorities in aoa. Since that time, we have been focusing on four objectives. Those objectives and our long-term care agenda build on one of the ready infrastructures that, should the Congress decide to have long-term care, can be quickly utilized. That is the aging network, the 57 State offices or units on aging, the 670 area agencies on aging, the over service providers who are already providing community-based services, and over 225 tribal organizations.
