L'apparition de la plainte souvent tardive impacte sérieusement la durée moyenne de vie des patients. Dans ce cadre, une approche précoce permettrait de prévenir, ralentir et éviter l'installation sévère du trouble. Cet essai thérapeutique s'articule autour de la pratique du Human Beatbox et la transmission d'informations sur les troubles de la déglutition dans la maladie de Parkinson. Le Beatbox est une technique vocale de production de sons et de rythmes dont l'intérêt sur la motivation et les mécanismes physiopathologiques encourage son utilisation pour les patients parkinsoniens. L'apprentissage de cette technique est relativement simple et a déjà montré ses effets en pratique clinique. Cette étude exploratoire vise à évaluer les effets de l'interventionsur la prise de conscience des troubles de la déglutition et le maintien ou l'amélioration des phases orales et laryngo-pharyngées. Cinq patients sans trouble dysphagique sévère ont suivi une thérapie de groupe d'une durée d'1h15 pendant neuf semaines consécutives, associée à des exercices journaliers. Des questionnaires d'autoévaluation et un examen quantitatif et qualitatif des capacités de déglutition avant et après le traitement ont été proposés. Les résultats montrent des améliorations à plusieurs niveaux : une diminution des symptômes spécifiques ressentis dans le quotidien des patients, une amélioration de la mastication, une augmentation du Temps Maximum de Phonation et de l'intensité vocale ainsi qu'un gain motivationnel. Cette étude présente des preuves préliminaires de l'effet bénéfique d'une thérapie incluant des exercices de Human Beatbox comme outil d'intervention précoce des troubles de la déglutition dans la maladie de Parkinson. Son efficacité reste à contrôler avec un groupe apparié dans le cadre de nouvelles études.

Impactant la communication et la qualité de vie, ces troubles sont pris en soin en orthophonie. Néanmoins, l'accès aux soins reste difficile Le télésoin offre une approche alternative et complémentaire aux thérapies actuellement proposées. Les objectifs de cette étude sont d'évaluer la faisabilité d'une intervention orthophonique de groupe en ligne de type éducation thérapeutique concernant les troubles de la parole et de la déglutition auprès de personnes atteintes de la MP, puis d'en explorer les retombées à court terme. Pour ce faire, deux groupes de quatre patients ont suivi six séances interventionnelles sur trois semaines. La réalisation de l'intervention et ses retombées sur la parole et la déglutition ont été ensuite évaluées grâce à une analyse qualitative et quantitative. Enfin, des retombées plus générales ont été mesurées qualitativement par une analyse thématique d'entretiens semi-dirigés organisés à la suite de l'intervention. Les résultats démontrent que l'intervention en ligne est réalisable sous certaines conditions. L'analyse quantitative ne démontre pas de résultats significatifs. Les résultats qualitatifs suggèrent que l'intervention participe à l'autonomisation et au bien-être des personnes atteintes de la MP.
