Book excerpt: Notre travail s’est attaché à identifier et décrire les motifs médicaux, environnementaux, socio-économiques et culturels à l’origine de l’insuffisance du suivi médical des femmes au début de leur grossesse. Nous avons mené une étude qualitative dans le département du Val de Marne, sous forme d’entretiens semi structurés réalisés avec vingt quatre femmes enceintes volontaires ; le critère d’inclusion était la non réalisation de la première échographie obstétricale avant quatorze semaines d’aménorrhée. Notre étude a mis en évidence trois catégories de motifs expliquant l’insuffisance du suivi : les caractéristiques socio-démographiques des femmes rencontrées (jeune âge, origine étrangère, situation sociale précaire, difficultés financières, absence d’emploi, logement précaire) ; le fonctionnement du système de soins (méconnaissance de l’existence des centres de protection maternelle et infantile, des dates auxquelles les examens doivent être réalisés, délais d’obtention des rendez-vous pour les différents acteurs du système de soins, non prescription de l’échographie par le médecin généraliste, refus de réalisation de l'échographie à une femme bénéficiaire de la CMU ou enceinte de plus de trois mois) ; le vécu de la grossesse (femmes n’ayant pas de projet de maternité, ayant initialement pensé effectuer une IVG, en échec de contraception, présentant des troubles du cycle, craignant l’examen gynécologique, retardant l’annonce de la grossesse à l’entourage, en difficulté de couple, isolées socialement) . Nous avons montré que ces catégories étaient souvent associées, avec des motifs multiples pour chacune des situations étudiées. Les réponses permettant d’améliorer le défaut de suivi doivent donc être globales, prenant en compte l’ensemble des problématiques pour pouvoir être efficaces
