Book excerpt: Although the physiology of the menstrual cycle appears clear and easily explained by a balance in the concentration of various sex steroid hormones, numerous details of its mechanism are still poorly understood and little is known about the relationship among clinical events, plasma hormone concentrations, molecular impacts on target tissues and their regulation. In the following chapters, the authors have attempted to establish a correlation between endometrial histology and well-understood physiologic events of the menstrual cycle. They have provided up-to-date information on the effects of various hormones and combinations of hormones on the endometrium. The interdependence of endometrial morphology, molecular biology, endocrinology and physiology, provides grounds for a better understanding of the complex mechanism of the menstrual cycle, and sheds some light on its pathophysiology. Such an approach adds ariother dimension to interpretation of many menstrual abnormalities and numerous aspects of infertility in women with normal physiognomies and apparently regular menstrual cycles. The pathologist must be aware of these new concepts since a knowledge of functional changes reflected in hormone serum levels and sex steroid receptor concentrations allows a mare detailed analysis and a better interpretation of the structural features of the endometrium. This information placed in the proper clinical context can help the gynecologist provide optimal therapy. The reader will find a valuable reference for a synthesis of clinical, morphological, and biochemical data related to the menstrual cycle and its aberrations.

The topics for trlese meetings have reflected the changing interests and backgrounds of the organizers. The recent rapidly increasing quantity of information on the uterine endometrium and its continuing relevance to fertility control stimulated the organizers to focus the 1979 workshop on this topic. The first four speakers provided a histological background for the molecular and biochemical studies described subsequently. The interaction of biochemist, physiologist, pathologist and physician as they focus their thoughts and talents on this limited area of the body is apparent in the dis cussions of presentations. This workshop was planned to provide in sight and a sense of proportion for the participants. For the reader, the record of the proceedings should serve as a useful reference to the state of the art in this area of research. The uterine endometrium is central in the successful implantation and development of the embryo. Examination of the physical relation ships, biochemical interactions and hormonal influences in this tissue could further advance the understanding of this pivotal tissue in the reproductive process. The depth of the understanding of endo metrial physiology should be reflected in our ability to better con trol this reproductive endpoint and to treat its dysfunction. The efforts of many individuals made these Proceedings possible.

Immense have been the preparations for me, Faithful and friendly the arms that have help'd me. Cycles ferried my cradle, rowing and rowing like friendly boatmen. For room to me stars kept aside in their own rings, They sent influences to look after what was to hold me. Before I was born out of my mother, generations guided me, My embryo has never been torpid, nothing could overlay it. -Walt Whitman, "Song of Myself" The womb is the seat of all mammalian life. In pregnancy, the uterus acquires this impor tance with the arrival of the fertilized egg, which takes up residence for periods ranging from about 2 weeks in the opossum to about 2 years in the elephant. The arrival of the embryo signals a crucial time for the establishment of pregnancy. For several days the blas tocyst remains free in the uterine lumen, where it depends on uterine secretions for its sur vival and differentiation. During this time, essential changes in the endometrium take place in preparation for attachment of the blastocyst and implantation. Early embryonic loss is an economic problem of global proportions in animal husbandry, where, in pigs and cattle for example, some 30% of all fertilizations fail to result in a pregnancy. In humans this figure may be even higher, and estimates of early spontaneous abortions range from 40 to 60% of all conceptions.

Book excerpt: The Organon Symposia have actually become a tradition, keeping up with exciting developments in reproductive medicine. The purpose of this symposium on "Fertiliza tion of the Human Egg in Vitro" was to bring together the stilllimited number of elinical specialists in the field and to stimulate another group of basic research people to exchange their experiences and knowledge, hopefully promoting elose cooperation between the two groups. It was a kind of scientific "first" that all research teams so far successful in achieving the birth of a healthy baby, fertilized in vitro came together at a workshop conference without a large audience of spectators and reporters, but with a small number of highly critical colleagues from the fields of basic reproductive physiology and comparative developmental biology. This atmosphere allowed for the elose exchange of results, hypotheses, diagnostic and therapeutic procedures, criticism, and respect, and created very productive discussions, all of which furthered the aim of the method: To help more childless couples to have their own babies by the ultima ratio procedure of in vitro fertilization and embryo replacement. The book that has emerged from this symposium will help to disseminate the great amount of information and experience gathered among the scientifically and clinically interested colleagues of many other hospitals and universities who could not be invited to the meeting. At the same time, it will prove that there is much more work to be done in the basic and clinical sciences of human embryology and reproductive biology.

Book excerpt: This monograph represents the eighth sponsored by the International Society for Biochemical Endocrinology. The topics should be of interest to basic research scientists, medical practitioners, and students of repro ductive biology. It complements our monograph published in 1979 on Structure and Function of the Gonadotropins. The monograph is organized in ten topic areas relative to the general theme of reproduction and contraception. There are several chapters in each area. Obviously, all aspects of each area could not be covered. An attempt was made to seek interesting basic research ideas and concepts that might in the future be applicable to fertility regulation. The topics are: interactions in gonadotropin regulation; GnRH analogues as contra ceptive agents; receptors in cellular localization of hormones; uterine and mammary receptors; germ-cell regulation and secretory proteins; control mechanisms and metabolic regulations; hCG peptides and anti sera as antifertility agents; ieutinization, oocyte maturation, and early pregnancy; steroids and cell growth; and finally, prostaglandins and cell function. The studies encompass many disciplines and techniques in anatomy, physiology, biochemistry, and endocrinology in animals and humans, both in vitro and in vivo. A conference of contributors was held in Maine at the Asticou Inn in Northeast Harbor during the week of September 9-13, 1979. The chapters as written for the monograph were presented for discussion by the participants, who were selected for their knowledge of, and contri butions to, this area of scientific investigation.
