Book excerpt: Introduction: L'accident ischémique transitoire (AIT) est une urgence diagnostique et thérapeutique. Le score ABCD2 est un outil pronostique évaluant risque de survenue d'un infarctus cérébral dans les 48 heures suivant un AIT. D'autres facteurs de gravité (FG) sont connus: sténose carotidienne, cardiopathie rythmique, AIT récidivant. Objectif: Eduquer et sensibiliser les médecins généralistes (MG) à l'AIT en introduisant le score ABCD2 en médecine générale comme outil pronostique et d'orientation des patients afin d'améliorer la prise en charge initiale des AIT. Matériels/Méthodes: Etude descriptive avec introduction du score ABCD2 et mise en place d'un formulaire auprès des MG en activité du bassin de Thau à remplir en cas de suspicion d'AIT. Recueil prospectif et rétrospectif des formulaires aux urgences de Sète. Les dossiers des patients ·orientés directement à l'UNV depuis la médecine générale n'ont pu être récupérés. Critères de jugement: orientation initiale du patient, délai entre l'apparition des symptômes et la première consultation médicale, niveau de risque (ABCD2 5 3 avec/sans FG, ABCD2 >= 4), orientation finale et délai de réalisation d'une imagerie cérébrale. Résultats: 22 patients ont été admis pour AIT aux urgences de Sète. 6 patients (27,3%) ont vu en consultation leur MG, 16 (72,7%) sont venus de leur propre chef. 90% ont consulté dans les 24 heures suivant le début des symptômes, 50% dans les 3 premières heures. 17 patients (77,3%) avaient un haut niveau de risque (ABCD2 4 ou ABCD2 s 3 avec facteurs de gravité), 5 (22,7%) un bas niveau de risque (ABCD2 >=3). Le principal FG présent a été la notion de récidive. 2 patients (9,1 %) ont consulté pour AIT récidivant sur la période. Tous les patients ont bénéficié d'une imagerie cérébrale, dans un délai de H +90 minutes pour les patients consultant dans les 3 premières heures, versus H+270 pour ceux consultant à plus de 24 heures. Pas d'amélioration de ce délai lorsque les patients sont adressés par leur MG (H+170). 2 patients (9,1%) ont été pris en charge à l'UNV. Conclusion: La place du médecin généraliste se situe en amont dans l'éducation et la sensibilisation des patients plutôt que dans la prise en charge initiale. Le score ABCD2 est un outil pronostique de stratification du risque -il'AVC après un AIT mais ne peut servir d'outil d'orientation des patients en médecine générale. Nécessité d'une étude avec un plus grand effectif sur une période d'observation plus longue pour étudier l'impact du score dans les pratiques
