Book excerpt: La maladie d'Alzheimer se caractérise par des troubles olfactifs et une perte d'autonomie instrumentale, et ce, dès le stade prodromal. Le but de notre étude était ainsi de vérifier l'existence d'un lien entre l'identification olfactive et l'autonomie puis de définir le type d'odeurs le plus altéré en fonction de la perte d'autonomie. Nous avons inclus dix sujets et évalué l'identification des odeurs grâce à une mallette olfactive spécifique à la maladie d'Alzheimer et à la culture française. Nous avons également évalué le niveau d'autonomie par le biais de l'IADL. Des données supplémentaires ont été recueillies grâce à un questionnaire portant sur l'identification d'odeurs écologiques et la survenue d'accidents domestiques. Nos résultats ont montré qu'il n'y avait pas de corrélation statistiquement significative entre le score d'identification olfactive et le score obtenu à l'IADL au sein de notre échantillon. En revanche, de manière qualitative, on constate que plus le score d'identification olfactive était élevé, plus l'autonomie des sujets présentant une maladie d'Alzheimer était préservée. De plus, on remarque que les sujets autonomes identifiaient mieux les odorants alimentaires que les sujets non autonomes, mais moins bien les odorants floraux et écologiques que leurs pairs. Ces résultats mettent en évidence une meilleure identification des odeurs écologiques par les sujets non autonomes par rapport aux sujets autonomes et donc l'importance d'utiliser ce type d'odeurs pour évaluer la perte d'autonomie. L'évaluation des capacités d'identification des odeurs écologiques potentiellement dangereuses, comme celles du gaz ou de la fumée, permettrait ainsi de dépister les situations à risque afin de limiter la survenue d'accidents domestiques.
