Book excerpt: Medical records documentation is burdensome for health care providers in terms of both the time and costs involved in their production. Recent advances in voice recognition technology have made it an alternative to transcription services. This pre-implementation study, conducted within the Department of Pathology and Area Laboratory Services at Walter Reed Army Medical Center, sought to determine if voice recognition technology could be leveraged to improve the production of its anatomic pathology reports. Work process analyses, a transcription services satisfaction survey, and a financial analysis were performed to determine if the time and costs to produce these reports could be reduced. The work process analyses showed the voice recognition system could substantially simplify the process of producing these reports from twelve to four steps. The satisfaction survey showed the pathologists were dissatisfied with the current transcription services. The foreign-born pathologists with accents were particularly dissatisfied with the accuracy, and younger pathologists were dissatisfied with the timeliness of producing the transcribed reports. Use of the voice recognition system could result in a cost savings of $520,000 over five years by eliminating the need for six medical records transcriptionist positions. These results indicate voice recognition could be used to reduce the time and costs involved in the production of the pathology reports, however, the results need to be confirmed following the implementation of the voice recognition system.

When combined with the speech recognition technology (SRT), EMR completeness has increased over methodologies without this technology. This research examined chart audit completion scores of physicians and scribes working within four Northeastern Ohio Emergency Services departments. SPSS® Statistics were used to perform a Repeated Measure Analysis using paired-samples t tests calculated to compare mean completion scores one month prior versus six months after SRT implementation. The mean completion score of pre-SRT implementation with and without the assistance of scribes was 5.5 ( sd = .8) and the mean completion score of post-SRT implementation without the assistance of scribes was 6.0 (sd = .9) indicating a significant increase from pre-SRT versus post-SRT implementation (t(17) = -3.9, p

GAO found that automated medical records offer the potential for great improvements in management of patient care. Automated records are far more accessible than paper records, more complete and accurate, can increase staff productivity, and may reduce operating costs. Factors which have impeded progress, include technology that is not fully developed or is considered too costly, potential misuse of automated information, and user resistance to automated systems. Other factors need to be addressed. At present, there is a lack of standardization in the ways data are collected and processed, which limits their usefulness for research. Automated records also raise questions about the security and privacy of sensitive information. In addition, the legal implications of automated patient records are still not clearly defined. In spite of these problems, there are significant benefits with automation. Solutions to the problems associated with automating medical records need to be found. Whether computers can support medical practice seems less the issue than how to bring about the development and use of the technology to do so.
