Les patients handicapés mentaux sévères n'ont pas accès à la communication verbale. Ceci rend difficile la prise en charge de leur douleur. Quelques outils ont été développés pour mieux l'évaluer et des recherches nous permettent de mieux comprendre la douleur chez ces patients. Cependant, en France selon le rapport du HCSP sur l'évaluation du troisième plan douleur, il est difficile de savoir comment est prise en charge la douleur des patients handicapés mentaux sévères. Pour essayer d'apporter une réponse, nous avons réalisé une étude descriptive auprès des familles de 111 patients handicapés mentaux sévères. La douleur est un problème médical important puisque 72 % des patients de l'échantillon souffrent d'épisodes douloureux. Il est sous-estimé et sous-traité. Un tiers des patients sont polymédiqués mais seulement 5 o/o ont un traitement antalgique. La douleur est traité le plus souvent par un médicament, à 50 % par un antalgique de palier 1 pour des douleurs majoritairement intense à très intense. Au-delà de ce constat attendu, notre étude nous laisse entrevoir des éléments de la prise en charge de la douleur des patients handicapés mentaux sévères qui sont déficients. L'évaluation et la prévention en sont les principaux. Nous constatons aussi l'importance des familles. Pour améliorer l'évaluation et la prévention de la douleur chez les patients handicapés mentaux sévères, il parait nécessaire d'améliorer la communication entre tous les intervenants auprès de ces patients. Nos réflexions nous mènent dans deux directions. L'une sur la formation notamment des familles sur la douleur et ses caractéristiques dans le handicap mental sévère. L'autre sur la mise en commun dans un dossier de tous les éléments nécessaires à la prise en charge de la douleur. Nos propositions peuvent se mettre en place dans le cadre du Plan douleur 2013-2017

Méthode : La recherche d’articles a été effectuée dans les bases de données CINHAL et PUBMED. Ils ont été mis en lien avec la théorie de la gestion des symptômes. Résultats : Les échelles les plus adaptées pour évaluer la douleur dans des services de soins aigus sont Algoplus et PAINAD. Les principales barrières rencontrées par les infirmières lors de la prise en charge de ces patients sont : la difficulté d’évaluer la douleur liée aux troubles cognitifs, la difficulté de donner des soins et des médicaments à cause de la confusion, les craintes liées aux effets secondaires des médicaments et le manque de temps. Discussion : Les infirmières devraient suivre une formation sur la douleur pour connaitre les traitements efficaces, leurs effets et les outils d’évaluation appropriés. Ces échelles permettent un suivi plus précis de la douleur. Une bonne gestion de la douleur a un impact positif sur le patient, les soignants et l’institution.
