Les patients porteurs d'une canule de trachéotomie peuvent présenter une altération de la déglutition par différents processus. L'identification précoce et précise des mécanismes physiopathologiques est indispensable pour limiter les complications respiratoires et accompagner le patient vers une reprise alimentaire orale progressive. Pourtant, il n'existe pas d'outil spécifique à l'évaluation de la déglutition du patient trachéotomisé en réanimation. Plusieurs objectifs ont conduit cette étude. Il s'agissait d'abord d'identifier les pratiques évaluatives des orthophonistes, d'objectiver un manque d'outil et de questionner l'utilité d'élaborer un support spécifique aux patients trachéotomisés. Elle visait également à concevoir un protocole d'évaluation à partir de données cliniques et théoriques et de le soumettre à une première mise en application par des orthophonistes cliniciens. La méthodologie utilisée repose sur une démarche d'analyse des pratiques professionnelles et de création d'outil. Deux questionnaires ont été adressés aux orthophonistes intervenant ponctuellement ou régulièrement en service de réanimation et/ou post-réanimation et prenant en soin des patients adultes trachéotomisés. La première enquête consistait en un état des lieux des pratiques et la seconde visait à évaluer la pertinence clinique de l'outil élaboré. Les principaux résultats ont révélé des discordances dans les pratiques d'évaluation, une absence d'outil spécifique et un intérêt clinique pour un tel support d'évaluation de la déglutition des patients trachéotomisés. Des disparités en termes de compétences et de connaissances dans la gestion des canules de trachéotomie ont également émergé. Par ailleurs, le protocole conçu est apparu pertinent et adapté à la pratique des orthophonistes de réanimation et post-réanimation. Enfin, les résultats se sont avérés plutôt encourageants pour les perspectives envisagées, et notamment la modification du présent outil et sa mise en application à plus grande échelle.

Forensic practice has forced me from time to time to consider rather more deeply this distressing syndrome, and to try and marshal my ideas in a form which would satisfy my colleagues in the legal profession. More than once I have been instructed to make a medico legal assessment of these unfortunate patients. The points which have concerned my lawyer friends have not been matters of diagnosis, or of morbid anatomy, or of etiology. The fac tual problem which has been put before me was to make some approximate assessment as to the expectation of life. Vague guess-work is unacceptable in such circumstances. What the lawyers require is a precise and dogmatic answer.
