Cette affection peut avoir de lourdes conséquences sur le fonctionnement global des individus, et cela, de façon chronique. Les facteurs influençant la survenue de ce trouble sont encore mal connus. Or, il est actuellement admis qu'un repérage et une prise en charge précoces peuvent en limiter la constitution et les conséquences. L'objectif principal de notre travail est l'étude des facteurs prédictifs de la survenue d'un état de stress post-traumatique, dans un groupe ayant subi un accident de la voie publique au cours d'un déplacement en bus lors d'un voyage organisé. Il s'agit d'une étude rétrospective à un an. Nos résultats ont mis en évidence une relation entre la survenue précoce d'une dissociation péritraumatique et la constitution d'un état de stress post-traumatique. Cet élément semble très intéressant en pratique clinique pour repérer de façon précoce les sujets à risque de PTSD et d'optimiser leur prise en charge. Par ailleurs, nous avons retrouvé des liens indirects entre les stratégies de coping, mises en place par les victimes au cours de l'évènement traumatique, et le PTSD. Nos résultats concordent avec les données de la littérature en soulignant le fait que les stratégies passives, telles que celles centrées sur l'émotion, l'évitement, l'imagination ou encore la culpabilité, sont des stratégies maladaptatives influençant la survenue d'un état de stress post-traumatique. L'étude de ces stratégies cognitives, encore peu explorées, semble être une piste intéressante dans l'étude du repérage des sujets à risque de PTSD. Une étude prospective permettrait de mieux cerner le lien existant entre les stratégies de coping et le syndrome de stress post-traumatique.

Book excerpt: L’Etat de Stress Post Traumatique est un terme récent apparu en 1980 dans le Manuel Diagnostic et Statistique des troubles mentaux (DSM III) qui a remplacé celui de névrose traumatique.Sa définition se complétant par la suite dans le DSM IV en 1994, l’ESPT est une pathologie complexe,souvent associée à d’autres comorbidités,et ayant de nombreuses répercussions dans la vie du sujet. Au cours des dernières décennies, de nombreux modèles différents ont tenté d’expliquer cette pathologie avec comme point commun cette notion de rupture dans la temporalité du sujet. Afin de mieux comprendre ses impacts sur les systèmes nerveux, endocrinien et immunitaire mis en évidence par de nombreuses études récentes et controversées, il était important de partir de la définition du stress. A travers un cas clinique, d’une part, j’ai voulu me focaliser sur l’impact cognitif que pouvait avoir l’ESPT notamment sur les troubles attentionnels, mnésiques. D’autre part, j’ai souhaité montrer la nécessité d’une prise en charge précoce et pluridisciplinaire de l’ESPT , en prenant comme exemple l’infarctus du myocarde, pathologie potentiellement mortelle. Par ailleurs, parmi les facteurs prédictifs de l’ESPT, il a été constaté que les troubles de personnalité constituent un élément essentiel dans la prévention de l’apparition de l’ESPT. Bien qu’un impact cognitif a pu être mis en évidence, il a pu néanmoins être discuté devant l’association de comorbidités psychiatriques,des pathologies médicales associées mais aussi par la prescription concomitante de psychotropes. Une évaluation pluridisciplinaire lors de l’apparition d’un évènement traumatique semble donc être nécessaire pour la prévention et une prise en charge optimale de l’ESPT

The task was to identify and analyze the scientific and professional social science literature pertaining to the psychological and/or behavioral dimensions of terrorist behavior (not on victimization or effects). The objectives were to explore what questions pertaining to terrorist groups and behavior had been asked by social science researchers; to identify the main findings from that research; and attempt to distill and summarize them within a framework of operationally relevant questions. To identify the relevant social science literature, the author began by searching a series of major academic databases using a systematic, iterative keyword strategy, mapping, where possible, onto existing subject headings. The focus was on locating professional social science literature published in major books or in peer-reviewed journals. Searches were conducted of the following databases October 2003: Sociofile/Sociological Abstracts, Criminal Justice Abstracts (CJ Abstracts), Criminal Justice Periodical Index (CJPI), National Criminal Justice Reference Service Abstracts (NCJRS), PsycInfo, Medline, and Public Affairs Information Service (PAIS). Three types of annotations were provided for works in this bibliography: Author's Abstract -- this is the abstract of the work as provided (and often published) by the author; Editor's Annotation -- this is an annotation written by the editor of this bibliography; and Key Quote Summary -- this is an annotation composed of "key quotes" from the original work, edited to provide a cogent overview of its main points.
