Méthodes : Cette étude cas témoins, prospective, multicentrique, a été réalisée en conditions de conduite écologique entre Juillet 2012 et Août 2014. Les patients inclus dans l'étude étaient issus de services hospitaliers gériatriques de trois villes de France : Reims, Strasbourg, Paris. Les participants sains ont été recrutés à partir d'associations de séniors. L'étude se déroulait en condition de conduite écologique et les performances de conduite étaient évaluées grâce à l'échelle NaDAS. Une régression linéaire multivariée pas à pas descendante a été utilisée. Résultats : Quarante et un participants ont été inclus dans l'étude : 20 présentaient une maladie d'Alzheimer au stade léger (MMSE ≥ 24), 21 étaient des sujets âgés sans troubles neurocognitifs. La consommation de psychotropes et le MMSE étaient statistiquement associés à une moins bonne performance sur le score de conduite des participants. Conclusion : Cette étude met en évidence l'impact des psychotropes sur le score de conduite automobile de sujets âgés avec et sans maladie d'Alzheimer au stade léger (MMSE ≥ 24).

Les médecins confrontés à ces pathologies connaissent des difficultés à évaluer l'aptitude à la conduite automobile. L'objectif principal de cette étude était de déterminer les facteurs associés à l'arrêt de la conduite automobile en HDJ mémoire. Matériels et Méthodes : Les données des patients consultant en hôpital de jour mémoire d'un centre universitaire ont été analysées rétrospectivement. Les patients inclus étaient âgés de plus de 65 ans, conducteurs actifs ou ayant arrêtés récemment de conduire (moins de 5ans). Résultats : Le groupe des conducteurs actifs était composé de 82 patients et le groupe arrêt de la conduite récente était composé de 45 patients. L'âge, les troubles cognitifs, les troubles de la marche, les pathologies articulaires, l'impossibilité de réaliser la station unipodale, un score IADL diminué et la non pratique d'activités de loisirs étaient retrouvés comme facteurs associés à l'arrêt de la conduite automobile. Conclusion : L'évaluation de l'aptitude à la conduite automobile chez les seniors doit prendre en compte non seulement les pathologies cognitives mais aussi les pathologies motrices et sensorielles. Les conséquences de l'arrêt de la conduite automobile sur la qualité de vie doivent aussi être considérées lors de cette évaluation. En France, il n'existe pas d'évaluation obligatoire systématique de la pratique de la conduite automobile chez la personne âgée ; la question reste posée d'une évaluation pluridisciplinaire de l'aptitude à la conduite automobile des seniors sans pour autant stigmatiser la personne âgée.

Dans le contexte d'une population vieillissante, l'autonomie du sujet âgé, très dépendante de son aptitude à conduire, doit être favorisée. Si la proportion des accidents impliquant des personnes âgées est faible par rapport à l'ensemble des accidents, la considération du facteur d'exposition multiplie le risque des plus de 65 ans jusqu'à 2,4 fois celui des 24-65 ans. Notre travail a porté sur l'étude, au cours de la maturation, de sujets sains, sélectionnés sur des critères médicaux et neuropsychologiques. Nos travaux s'articulent autour de données objectives relevées en conduite simulée et de données subjectives portant sur la conduite automobile quotidienne. Outre l'intérêt porté aux effets des facteurs endogènes et exogènes, nous nous intéressons particulièrement aux stratégies adaptatives des sujets les plus âgés. Les résultats subjectifs montrent que les sujets âgés manifestent un important besoin de se servir de leur véhicule, qu'ils sont conscients et confiants en leurs capacités, mais cependant fragilisés dans l'interaction avec les autres conducteurs, vécue comme angoissante. La stratégie adaptative la plus couramment décrite est l'augmentation de la distance inter-véhicules qui apparaît dès l'âge moyen. Les résultats obtenus en simulation montrent que les personnes âgées modulent leurs comportements selon la difficulté des conditions de conduite. Elles se servent à bon escient de l'éclairage autoroutier et se montrent plus capables que les sujets jeunes à assumer une conduite prolongée de nuit. Le vieillissement différencie les comportements de conduite entre les hommes et les femmes indépendamment de l'expérience de conduite des sujets. La situation simulée a permis de révéler, chez les sujets âgés, des comportements non adaptatifs à la conduite automobile qui trouvent leur origine dans les caractéristiques physiologiques, psychologiques et comportementales accompagnant le vieillissement. Nous proposons de considérer que les comportements du conducteur âgé traduisent l'expression d'une adaptation à la conduite automobile, de ses aptitudes, de celles qu'il se prête ou qu'il est prêt à assumer, et non l'expression d'une volonté à réduire le risque qu'il pourrait représenter pour lui-même ou en matière de sécurité routière.

Objectif : nous voulons étudier la relation entre la prise d’hypnotique et la conduite automobile chez les personnes âgées. Méthode : nous avons réalisé une revue systématisée de la littérature en interrogeant les bases de données PubMed et Cochrane, et présenté un projet de recherche clinique. Résultats : neuf études ont été retenues pour cette analyse. Il semble exister un lien entre benzodiazépines et augmentation du risque d’accident, plus la demi-vie s’allonge et plus la dose est importante. Il existe peu d’études évaluant le retentissement des apparentés des benzodiazépines sur la conduite automobile chez la personne âgée. Projet de recherche : nous proposons un projet de recherche permettant d’évaluer le retentissement du zolpidem sur la conduite automobile chez la personne âgée. Conclusion : les personnes âgées constituent une population à risque, au vu de leur polymédication et consommation d’hypnotiques, pouvant représenter un problème de santé publique du fait de l’accroissement de conducteurs âgés, ce qui nécessite des études supplémentaires.
