Book excerpt: Cette étude qualitative a pour but d'explorer par le biais d'entretiens semi-directifs comment les soignants travaillant en clinique spécialisée dans le traitement des douleurs chroniques établissent une alliance thérapeutique avec les patients atteints de douleurs chroniques médicalement inexpliquées, enrayés dans leur problématique de souffrance. Si croire à la douleur est primordial dans la prise en soins, les résultats ont révélé que les soignants adoptent des attitudes spécifiques dans leur approche des patients qu'ils identifient comme atteints de souffrances psychosociales. L'empathie est une des primes qualités pour prendre en soin des personnes en mal de reconnaissance et besoin d'écoute. Il est recommandé de valider la douleur et de provoquer ce que les soignants nomment " prises de conscience " pour favoriser dans le respect des malades la découverte d'un sens et de stratégies d'adaptations à la douleur, autrement dit en considérant leur façon de vivre et de se représenter leur douleur. Les soignants permettent ces prises de conscience au décours du groupe de parole " douleur " où ils tentent le pari de décentrer les patients de leur douleur mais aussi par l'évaluation de la douleur, à la fois validation d'un symptôme amené par une plainte trop souvent méprisée de l'entourage et ouverture possible sur les souffrances de vie. Ainsi l'alliance thérapeutique est acquise autour d'un savoir faire, celui d'accueillir la plainte sans se laisser déstabiliser par sa chronicité. Les soignants, dont les représentations de la douleur chronique médicalement inexpliquée rejoignent celles de la somatisation de la douleur ont identifié des attitudes à proscrire mais aussi des ressources inhérentes à l'élaboration de cette alliance avec les patients qui n'identifient pas toujours de lien entre vécu et douleur.
