Book excerpt: Introduction: En 2012, en France, l’Hospitalisation à Domicile comptait 300 établissements prenant en charge plus de 11 000 patients par jour. La volonté politique actuelle est de consolider l’HAD dans l’offre de soins avec l’objectif de doubler le nombre de séjours dans les cinq ans à venir. Le médecin généraliste est présenté comme le pivot de ce système de soins à la frontière entre la médecine hospitalière et la médecine ambulatoire. Matériel et méthodes: Nous avons effectué une enquête qualitative, avec la réalisation de trois focus groups, réunissant des médecins généralistes exerçant en Picardie afin de tenter d’analyser les problèmes qu’ils rencontrent au sein du système d’HAD et d’élaborer des pistes d’amélioration. Résultats: L’analyse de leurs propos a mis en évidence leurs difficultés à s’intégrer dans le fonctionnement de l’HAD, des problèmes de communication et de coordination, une lourdeur administrative et la complexité à gérer ces prises en charge chronophages. Discussion: La participation initiale du médecin traitant à la décision d’hospitaliser à domicile le patient, le maintien du personnel paramédical libéral habituel, la clarification du rôle des différents intervenants et la simplification des démarches administratives apparaissent comme des possibilités de perfectionnement du fonctionnement de l’HAD pour les médecins traitants.Conclusion: La prise en compte des difficultés rencontrées par les médecins généralistes est un facteur indissociable au développement de l’HAD. D’autres enquêtes qualitatives basées sur des entretiens de groupes des autres intervenants permettraient d’envisager des solutions globales à l’amélioration de la coordination.
