Book excerpt: Le nombre de consultation dans les services d'accueil des urgences (SAU) hospitaliers ne cessent d'augmenter. Cette augmentation concerne principalement les consultations sans hospitalisation, relevant de prime abord de la médecine générale. L'objectif de cette étude était de comprendre pourquoi les patients relevant d'une consultation de médecine générale venaient au SAU sans avoir pris un avis médical préalablement? Les patients se présentant à des horaires d'ouverture de médecine de ville et dont le motif de venue relevait de la médecine générale ont été interrogés. Le questionnaire portait sur des données socio-économiques, leur parcours de soin et leur motif de consultation, ainsi que leurs habitudes d'utilisation des services de soins primaires. Une seconde partie du questionnaire a été rempli a postériori après analyse du degré de la classification clinique des malades des urgences (CCMU). L'analyse a porté sur 87 questionnaires remplis et analysés au cours d'entretien individuel. Les motifs rapportés par les patients étaient en premier lieu une impression de gravité de leur pathologie (20%) et le fait de ne pas avoir un accès rapide auprès de leur médecin généraliste (20%). 14% déclaraient n'avoir aucun médecin généraliste et venir aux urgences pour tout problème médical. Ces résultats sont en accords avec le fait que 60% ne connaissent aucun organisme dispensant des soins d'urgences autre que les SAU. L'analyse des dossiers montre que 51% des patients ont été classés en degré 1 de la CCMU, donc relevant simplement d'une consultation généraliste sans examen complémentaire ; et 33% en degré 2 donc nécessitant des examens complémentaires pouvant être réalisés en ville. Cette étude a permis de montrer que la majorité des patients ayant un recours spontané aux urgences sans avis médical préalable consultait de manière inappropriée. Leur prise en charge aurait pu être réalisée en ambulatoire et ne nécessitait ni SAU ni hospitalisation. Pour répondre à la demande de désengorgement des SAU, de nombreuses alternatives ont été créées (SAMI, Maison médicale de garde, téléconsultation, maisons et centres de santé pluridisciplinaires, permanence des soins) qui doivent être encore développés à l'avenir et faire l'objet d'une information des patients. Cette évolution permettrait de limiter la crise dont souffre l'hôpital public et permettrait de rendre aux urgences hospitalières sa vocation première : prendre en charge les urgences vitales.

Les usagers désirent une médecine rapide, disponible à toute heure avec un accès quasi immédiat au plateau technique. L'objectif de notre étude était d'analyser la proportion de patients qui consulte aux urgences sans recours préalable au médecin traitant pour des motifs relevant de la médecine générale. Il s'agit d'une étude bicentrique, par questionnaire, réalisée sur une semaine, incluant tous les patients majeurs consultants au centre hospitaliser Tenon et au centre hospitalier du Sud Francilien, classé CIMU 4 ou 5 à l'arrivée. Soixante-cinq pourcents des consultants n'ont pas essayé de joindre le médecin traitant. Soixante-dix-huit pourcents des patients ont jugé leur problème de santé urgent avec un délai de consultation après l'apparition des symptômes inférieur à 24h. Le motif de consultation était principalement médical. Les raisons de l'absence d'appel du médecin traitant était : l'absence de consultation en urgence, le sentiment de gravité ressentie, le fait de ne pas y avoir pensé et la fermeture du cabinet. Ils ont consulté aux urgences afin de voir rapidement un médecin et être soulagé de la douleur. Soixante-quinze pourcents des patients ne connaissaient pas la MMG et 58,3% était prêt à y consulter. L'enjeu essentiel semble être l'information et l'éducation des usagers sur le rôle primordial du médecin traitant et la nécessité du recours.

Matériel et Méthode - Étude prospective monocentrique durant quatre semaines basée sur un auto-questionnaire standardisé, adapté à !"horaire de consultation. distribué aux patients consultant sans avis médical préalable au service d'accueil des urgences de l'hôpital de Pontarlier. Chaque questionnaire est également composé d'un volet médical. Résultats - 232 patients ont répondu à l'enquête. Après exclusion des patients dont le recours aux urgences était justifié, 163 patients ont été considérés comme relevant d'une prise en charge ambulatoire par le médecin urgentiste, correspondant à 11,8% des admissions sur la période de l'étude. Ces patients sont plutôt des hommes jeunes, actifs et vivant à proximité de l'hôpital. Ils consultent majoritairement la journée, pendant les heures d'ouverture des cabinets de ville. Les causes principales du recours sont un besoin supposé d'examens complémentaires et une prise en charge rapide, notamment de la douleur. Sur les horaires de garde, 85% de nos patients n'ont pas été régulés par la permanence des soins ambulatoires, la moitié ne connaissaient même pas son existence. Pourtant, 70% auraient accepté une consultation auprès d'un médecin généraliste. Conclusion - Les résultats de notre étude incitent à entreprendre des actions de communication et d'éducation de la population. Il serait également nécessaire de faciliter la prise de rendez-vous en cabinet de ville pour les soins non programmés. La création d'une maison médicale de garde à proximité du service pourrait être très pertinente et bien acceptée par les patients qui seraient susceptibles d'y consulter ou d'y être réorientés.

The 2018 TZG updates and expands on the guidance in the one previous jointly-developed, zoonoses-specific guidance document: the 2008 Tripartite “Zoonotic Diseases: A Guide to Establishing Collaboration between Animal and Human Health Sectors at the Country Level”, developed in WHO South-East Asia Region and Western Pacific Region. The 2018 TZG supports building by countries of the resilience and capacity to address emerging and endemic zoonotic diseases such as avian influenza, rabies, Ebola, and Rift Valley fever, as well as food-borne diseases and antimicrobial resistance, and to minimize their impacts on health, livelihoods, and economies. It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals. The 2018 TZG provides relevant country ministries and agencies with lessons learned and good practices identified from country-level experiences in taking OH approaches for preparedness, prevention, detection and response to zoonotic disease threats, and provides guidance on multisectoral communication, coordination, and collaboration. It informs on regional and country-level OH activities and relevant unisectoral and multisectoral tools available for countries to use.
