Objectif : Identifier, évaluer et synthétiser les freins qui ont un impact sur la mise en œuvre des rôles des infirmières de pratique avancé dans les établissements de soins primaires. Méthode : Une revue de littérature systématique a été réalisée et rapportée conformément selon PRISMA. Quatre bases de données (Cochrane Library, Web of Science, Pubmed, et Embase) ont été consultées pour identifier les études publiées en anglais et en français entre juillet 2017 et septembre 2022. La sélection des études et l'évaluation méthodologique ont été effectuées par deux examinateurs de manière indépendante. Le codage déductif des freins et des leviers a été entrepris à l'aide de la classification de Yorkshire. Nous avons utilisé un codage inductif pour les freins ou les leviers qui ne pouvaient pas être classés à l'aide de codes prédéfinis. Les désaccords ont été réglés par la discussion. Les données descriptives ont été retranscrites dans différents tableaux, et les principaux résultats pour les freins et les leviers ont été rassemblés dans une synthèse narrative. Résultats : Une recherche systématique a identifié 17061 enregistrements potentiels, 229 résumés ont été examinés et 1 10 textes intégraux ont été récupérés. Soixante-huit articles répondaient aux critères de sélection. L'implantation des IPA rencontre des freins variables selon l'avancement de leur implantation. Les facteurs liés à l'équipe étaient le frein les plus fréquemment signalés. Les facteurs « procédures et politique » et le « contexte politique externe » étaient également les freins les plus fréquemment signalés. Conclusion : La prise en compte des freins à l'implantation afin de mettre en place des leviers antagonistes est nécessaire au succès de l'implantation des IPA. L'établissement de relations de collaboration avec d'autres professionnels de la santé et la négociation du rôle sont essentiels au succès de la mise en œuvre du rôle d'infirmière de pratique avancée dans les pays à implantation récente.
