Book excerpt: Introduction : Les fractures articulaires de base de 1er métacarpien engagent le pronostic fonctionnel de la main lorsqu'insuffisamment traitées. Entre broches, vis ou plaques vissées, le traitement chirurgical est non consensuel. Des désaccords persistent quant à l'exigence de réduction anatomique, les complications à craindre. L'objectif de cette étude est d'analyser de manière rétrospective le devenir de patients ayant présenté ce type de fracture. Matériel et Méthodes : 41 patients ont été étudiés sur des critères visant à évaluer la satisfaction post-opératoire, un questionnaire visant à évaluer l'activité du pouce intégré au membre supérieur, des mesures cliniques de force et de mobilité articulaires visant à évaluer la fonction du pouce, ainsi qu'une mesure radiologique de l'intégrité de l'articulation Trapézo- Métacarpienne avec évaluation de la consolidation osseuses et l'apparition d'une éventuelle rhizarthrose. Résultats : Les moyennes des douleurs de repos et d'effort sont de 0,7/10 et 2/10. La sensation subjective moyenne de perte de force du côté opéré par rapport au côté controlatéral est de 12,2%. Le score DASH moyen est de 8,8/100. La satisfaction globale moyenne de de l'intervention est de 87%. Les forces de préhension moyennes au Tip-pinch test du côté opéré et du côté controlatéral sont de 5,9 Kg et 6,5 Kg. Les angles d'abduction moyens du côté opéré et du côté controlatéral sont de 40,5° et 44,6°. Les score d'opposition du pouce moyens du côté opéré et du côté controlatéral sont de 9,5/10 et 10/10. La consolidation fut complète dans 100% des cas. Les patients présentaient une arthrose radiographique dans 58,54% des cas, mais sans retentissement clinique. Conclusion : Le brochage percutané intermétacarpien semble offrir un optimum en termes de : rapidité d'exécution, coût, résultats (consolidation, sédation des douleurs, restauration de l'anatomie articulaire, résultats fonctionnels et satisfaction du patient à long terme).

Numerous new implants and instruments are presented in this, the second edition. In 4.0-, 3.5- and 2.7-mm screws the hexagonal socket has definitively superseded the Phillips head, while mini-implants have been modified and a 1.5-mm screw introduced. A number of new plates and implants have been introduced and have long since proved their value. At the same time new techniques, described here, have been developed and applied. The tension-band wire is included, despite the fact that it does not consist of specific AO implants, and use of the Kirschner wire is also illustrated and emphasized whenever the authors consider that this simple method is still the best available. Alterations in the organization of the book have been necessary: the shoulder, the forearm and the knee are treated in separate chapters, and other sections have been extended. With regard to clinical-radiological examples, new typical situations are described and documented. The case studies from the first edition have been recontrolled in all instances in which it was feasible to do so. In this way many valuable late results, 9-12 years after internal fixation, were obtained. We found that almost all joints had remained stable after healing of an articular fracture; so-called late arthrosis is rare in the peripheral skeleton.
