Book excerpt: Introduction: Du fait d'une augmentation de la fréquentation des services d'accueil des urgences et de la complexité de la prise en charge des personnes âgées, il est recommandé de privilégier les entrées directes en court séjour gériatrique. Objectif principal: Identifier les facteurs déterminants la décision du médecin généraliste d'hospitaliser le patient âgé par une entrée directe en gériatrie ou par les urgences à Dinan. Matériels et méthode: Nous avons réalisé une étude qualitative auprès de 13 médecins généralistes ayant hospitalisé un patient âgé de 75 ans et plus en court séjour gériatrique via une entrée directe ou par les urgences. Résultats: Les entrées directes concernaient des pathologies gériatriques n'engageant pas le pronostique vital. La disponibilité des lits, le manque de temps étaient des freins aux entrées directes. Le numéro direct créait un lien entre les médecins hospitaliers et les médecins libéraux. Les médecins généralistes regrettaient le manque d'informations sur la filière gériatrique, et d'échanges avec les médecins hospitaliers, empêchant une utilisation optimale de la filière. Conclusion: Le parcours de soin du patient âgé doit être abordé dans sa globalité, et intégrer la dimension de prévention du vieillissement pathologique et de la dépendance; dans l'objectif de fluidifier la filière gériatrique. Renforcer les soins de santé primaires, valoriser la communication ville-hôpital, et la coordination des soins sont les enjeux actuels de la santé publique.
