L'alcool peut avoir à la fois des effets protecteurs et des effets nocifs sur la santé cardiovasculaire, l'identification des mécanismes biochimiques qui pourraient expliquer ces effets paradoxaux est nécessaire. C’est le vin rouge qui présente l’impact le plus positif sur la santé. L’alcool qu’il contient, issu de la fermentation du fruit mûr, possède une assimilabilité particulière. L’éthanol a lui seul a beaucoup d’action sur ces mécanismes biochimiques, mécanismes directement impliqués dans la survenue de phénomènes cardiovasculaires, et va donc exercer de nombreux effets bénéfiques sur la prévention de l’athérosclérose, tant au niveau du métabolisme lipidique, que sur l’hémostase. Mais dans le vin l’alcool n’est pas le seul composant à avoir démontré ces capacités protectrices. La quantité importante de polyphénols, présents dans le vin rouge notamment, joue un rôle primordial dans cette protection observée chez les buveurs modérés. Dans ce fabuleux mélange, l’alcool et les polyphénols vont agir en synergie contre l’apparition des maladies cardiovasculaires. Tout au long de cette thèse, je me suis aperçue que dans beaucoup d’études, le lien avec le mode de vie des personnes étudiées comme l’alimentation, n’est pas toujours fait. Cependant il est indispensable de le prendre en considération. Les fameux effets protecteurs de ce doux breuvage qu’est le vin sont inscrits dans une hygiène de vie se rapprochant de celle de nos ancêtres et du monde méditerranéen. Son action s’inscrit dans le cadre d’une alimentation équilibrée et d’un mode de vie sain. Aujourd’hui le vin ne peut être considéré comme un médicament mais sa consommation modérée (3 à 5 verres par jour) peut s’avérer bénéfique pour la santé. En oubliant pas que tout est une question de dose, la frontière entre le bénéfice et le poison est mince.

L'étude de la composition phénolique des vins de cépages (60 échantillons) par HPLC avec détection UV ainsi que l'estimation de la dose quotidienne de polyphénols ingérée chez l'homme pour une consommation modérée de vin de cépage montre que la prise de composés phénoliques est 10 fois plus importante pour les vins rouges (400,2mg/personne/jour) que pour les vins blancs ( 44,1mg/personne/jour). La prise la plus élevés de catéchines est obtenue pour les cépages : Egiodola, Grenache et Cabernet Sauvignon). Les monoméres ( catéchine et épicatéchine) peuvent représenter jusqu'à 40% du total des catéchines ingérées. L'élaboration d'un vin blanc de chardonnay " vinifié en rouge " a permis d'obtenir un gain important du taux de composés phénoliques totaux ( 4,26 fois) et de catéchines par rapport à la vinification traditionnelle. L'appréciation de son activité antioxydante par rapport à d'autres types de vins de cépages ( 54 échantillons : rouges, rosés, blancs secs ou liquoreux) du Sud de la France donne une valeur de capacité antioxydante de 13,8mmo1/L comparable à celle de certains vins rouges. L'analyse de différents vins de cépages et de raisins ( Sémillon, Sauvignon) à différents stades de pourriture noble a montré la présence d'oligoméres de stilbénes ( pallidol : dans des vins élaborés avec macération de la rafle ; viniférine : sur vins rouges et vins blancs liquoreux). L'effet de la consommation du vin blanc de chardonnay enrichi en composés phénoliques ; complet ou désalcoolisé sur des rats diabétiques ( modèle streptozotocine) déficient en insuline permet de restaurer les capacités antioxydantes du plasma à un niveau non significativement différent de celui des animaux non-diabétiques.

Le vin contient des constituants spécifiques qui lui donnent sa personnalité comme des composés aromatiques ou de nombreux composés phénoliques subdivisés en plusieurs groupes (acides phénols, acides cinnamiques, stilbènes, tanins, anthocyanes, flavonols) qui jouent un rôle fondamental dans la perception sensorielle du produit. De nombreuses études portent sur un composé en particulier, le Resveratrol. C'est une molécule du groupe des stilbènes qui, par ses nombreux effets bénéfiques sur la santé, se promet un bel avenir au niveau pharmacologique. Depuis très longtemps les chercheurs ont essayé de découvrir le mystère de cette boisson dont la consommation modérée entraîne des effets bénéfiques indiscutables. Ainsi, de nombreuses études portant sur les effets positifs du vin sur la santé ont fleuri : il existerait une action protectrice du vin contre les maladies cardiovasculaires qui diminuerait le risque d'infarctus et de thromboses vasculaires cérébrales.

The report makes extensive use of the Iraq Household Socio-Economic Survey, the first nationwide income and expenditure survey since 1988. IHSES data is complemented income and expenditure data from a wide range of other measures of living standards, allowing us to analyze living standards in a holistic way. The analysis presented here was performed with two main goals first, to inform the Government s Poverty Reduction Strategy; and second, to serve as a baseline for future assessments of changes in living standards and the identification of critical issues for deeper examination. Iraqi living standards have two unusual characteristics. First, they have fallen over the past generation. Second, they feature surprisingly little inequality. These characteristics are both rooted in Iraq s recent history of authoritarian government, war, military occupation, insurgency, and civil strife leading to infrastructure destruction and population displacement. There have been few opportunities for individuals to prosper from professional or entrepreneurial activities. Decades of neglected investment have resulted in deterioration of social services and economic infrastructure. Consequently, individuals have lacked capabilities to prosper and an investment climate conducive to prosperity. School enrollment and life expectancy have declined. Extremely low returns to education reflect the combination of poor educational quality and lack of employment opportunities. In terms of economic infrastructure, access to reliable electricity and water, and even access to paved roads are low, are further reflections of decades of neglect. While the upper end of the distribution has been pulled down by a lack of opportunities, the lower end has been supported by direct government provision of food. The Public Distribution System (PDS) provides 85 percent of food needs. While PDS has been useful as a safety net for the poor and the vulnerable, the system is expensive, inefficient, and fiscally risky. Indeed, PDS food rations account for a far greater share of public spending than does education or health. Going forward, Iraq faces two main challenges. First, although Iraq does not have to develop from scratch, it faces a formidable challenge in re-development. Second, a shift by the Government is required from direct provision of basic subsistence toward investment in human capacities. The Government can provide an enabling environment through investments in economic infrastructure and services to business and citizens, thus allowing the population to make productive use of education and their own labor. Both challenges are now being taken up by the Poverty Reduction Strategy, which articulates a detailed set of required actions and outlines priorities for government spending.
