Book excerpt: Avec l'arrivée des antiprotéases utilisées en trithérapie depuis 1996, les patients vont devoir apprendre à vivre avec leur VIH et entrer dans une maladie chronique. De nombreux articles vont alors montrer l'importance de l'observance thérapeutique (respect des prescriptions médicales) chez les personnes séropositives. Nous souhaitons montrer dans cette étude l'importance en particulier des déterminants psychosociaux de l'observance. Cette recherche réalisée en 2002, réunie 127 patients VIH (âgés de 18 à 65 ans) qui ont participé à une enquête par questionnaire lors de leur consultation trimestrielle à l'hôpital de Metz. Les sujets devaient répondre à une échelle de compliance (Tarquinio, Fischer, Grégoire, 2000), un questionnaire de représentation de la maladie (IPQ, Weinman), un questionnaire de représentation du traitement (BMQ, Horne), une échelle de valeurs (Fischer, 2002), une échelle de satisfaction au traitement et une fiche signalétique. Nous avons procédé à des analyses multidimensionnelles (ACP, régressions multiples, et équations structurales), afin de mettre en évidence les variables principales sous-jacentes aux échelles. Les résultats révèlent que les déterminants psychosociaux tels que les croyances des patients, leurs valeurs, les facteurs environnementaux et la relation médecin-malade, sont des éléments qui influencent l'observance thérapeutique. Les facteurs principaux en lien direct avec l'observance des patients, sont leurs croyances par rapport au traitement, la satisfaction du traitement, la confiance accordée au médecin, le soutien social, la durée du traitement et de la maladie et la communication sincère. Ces résultats sont consistants avec la théorie du modèle de Leventhal sur l'autorégulation de la maladie, et permettrons aux médecins d'ajuster leurs conseils et les traitements en tenant compte de ces différents déterminants psychosociaux, afin d'améliorer l'observance des patients VIH.

Book excerpt: Introduction : La mortalité et la morbidité liées au VIH ont considérablement diminué depuis l'introduction du traitement antirétroviral hautement actif (HAART). Cependant, le succès thérapeutique est lié à une bonne observance. La connaissance du niveau d'observance et des facteurs qui y sont associés, sont des paramètres importants dans la prise des décisions en vue de l'amélioration du suivi des patients. Objectif: Dans la présente étude, nous avons évalué le niveau d'observance et déterminer les facteurs associés à la mauvaise observance, chez les patients sous HAART suivi au Centre National Hospitalier et Universitaire à Cotonou au Bénin. Méthodologie : C'est une étude transversale qui a concerné 310 patients recrutés de manière successive. La mesure de l'observance a été basée sur l'auto rapportage. Deux échelles de mesure ont été utilisées pour évaluer l'observance sur les quatre derniers jours précédant le recrutement. Une échelle quantitative (prise d'au moins 95% des pilules prescrites) et une échelle qualitative (prise d'au moins 95% des pilules prescrites et le respect constant des heures de prise). Un modèle de régression binomiale a été construit pour déterminer les facteurs associés à la mauvaise observance, les rapports de proportion et les intervalles de confiance à 95%. Le niveau alpha a été de 0.05. Résultats : Sur les 310 patients recrutés, 62% étaient des femmes. Le nombre moyen de comprimé à prendre par jour était de 3. Selon l'échelle quantitative, la prévalence de la bonne observance était de 93%. Par contre avec l'échelle qualitative, la prévalence de la bonne observance était moins élevée : 59%. En analyse multivariée avec la régression binomiale, quatre facteurs étaient significativement associés à la mauvaise observance. Il s'agit de: la consommation d'alcool (RR=I, 49 ; IC95% 1,14- 1,96), l'absence de soutien social (RR=1,50 ; IC95% 1,14- 1,95), l'inquiétude de décéder du VIH (RR=1,55 ; IC95% 1,13- 2,12), et la durée du traitement de plus de un an (RR=1,34 ; IC95% 1,01-L77). Conclusion : Sur le plan quantitatif, le niveau d'observance était très bon et comparable ou supérieur aux résultats observés dans les pays industrialisés. L'observance qualitative reste par contre un défi. La maîtrise des facteurs psychosociaux chez les patients pourrait permettre de l'améliorer. Toutefois nos résultats devraient être validés par une étude comparant le niveau de la charge virale à celui de l'observance.

Méthodes: L'enquête a porté sur 138 patients sous traitement antirétroviral depuis au moins 6 mois, qui ont répondu à un questionnaire d'auto-évaluation des facteurs facilitateurs et des barrières à l'observance. Résultats: Le pourcentage de non-observance a été estimé à 29,71 % et l'échec thérapeutique retrouvé était de 45,65%. Différents facteurs démographiques, culturels et sociaux que nous évaluons expliquent cet échec. Conclusion: La compréhension des facteurs associés à l'observance afin de pouvoir l'augmenter est devenue un enjeu majeur de la recherche tout comme la promotion des interventions d'éducation thérapeutique pour faire connaître aux patients leur maladie et leur traitement, non seulement au moment de l'initiation de celui-ci mais aussi pendant toute la durée du traitement

Méthode : Dans une étude transversale analytique qui s'est déroulée durant 5 mois à l'hôpital Albert Schweitzer à Lambaréné, l'observance a été évaluée par la combinaison de deux méthodes : l'entretien médical et la ponctualité des rendez-vous. Résultats : Parmi les 90 patients recrutés, 80% étaient des femmes, la moyenne d'âge était de 42 ans, et 66% n'étaient pas observants. Les principales raisons de la non observance étaient : l'incompréhension de la maladie (60%), le déni de la maladie (32%), l'accessibilité financière (32%), la mauvaise prise en charge médicale (30%). Les patients plus vieux étaient plus observants (IC 95%, OR=1-1,1, p=0,044). L'ancienneté de la maladie était, quant à elle, un facteur de non observance lorsqu'elle tend à s'allonger : entre 1 et 5 ans d'ancienneté (IC 95%, OR=0,02-0,028, p=0,001), et plus de 5 ans d'ancienneté (IC 95%, OR=0,03-0,37, p=0,001). Conclusion : Une prise en charge médicale globale et de meilleure qualité est nécessaire pour améliorer l'observance, en s'appuyant sur l'éducation thérapeutique et l'appui psychosocial.

Drawing on his experiences as a physician and anthropologist in Burkina Faso and Côte d’Ivoire, Vinh-Kim Nguyen focuses on the period between 1994, when effective antiretroviral treatments for HIV were discovered, and 2000, when the global health community acknowledged a right to treatment, making the drugs more available. During the intervening years, when antiretrovirals were scarce in Africa, triage decisions were made determining who would receive lifesaving treatment. Nguyen explains how those decisions altered social relations in West Africa. In 1994, anxious to “break the silence” and “put a face to the epidemic,” international agencies unwittingly created a market in which stories about being HIV positive could be bartered for access to limited medical resources. Being able to talk about oneself became a matter of life or death. Tracing the cultural and political logic of triage back to colonial classification systems, Nguyen shows how it persists in contemporary attempts to design, fund, and implement mass treatment programs in the developing world. He argues that as an enactment of decisions about who may live, triage constitutes a partial, mobile form of sovereignty: what might be called therapeutic sovereignty.
