Book excerpt: Introduction : La prise en charge des rhumatismes inflammatoires dans la population gériatrique est essentielle, du fait que la douleur, la raideur et les déformations articulaires peuvent diminuer l'état fonctionnel des patients âgés et depuis l'introduction des biothérapies, la qualité de vie s'est nettement améliorée pour les patients atteints de ces pathologies. Ces traitements ont changé la prise en charge et l'évolution des patients en termes de morbidité et de mortalité, mais ils sont malheureusement associés à plusieurs effets secondaires. Objectifs : évaluer le nombre et typologie de complications aiguës survenant sous biothérapie chez une population atteinte de rhumatismes inflammatoires chronique classée gériatrique et chez une population non gériatrique, avec l'ajout d'une étude croisée avec des patients gériatriques non traités par biothérapies, afin de comprendre si les complications observées en terme d'infections ou d'autres effets secondaires sont liées à l'âge, aux comorbidités des patients ou simplement aux biothérapies. Matériels et méthodes : étude observationnelle rétrospective bi-centrique réalisée dans le cadre de cette thèse, qui inclut 162 patients au total (BIO-AGE). Les patients ont été classés en 3 groupes, afin de comparer la survenue des complications et comprendre si ces complications sont associées à l'âge, aux comorbidités ou aux biothérapies. Le groupe 1 comprend 40 patients âgés (>65) sous biothérapie, le groupe 2 comprend 67 patients non âgés (65) sous biothérapie et enfin le groupe 3 comprend 55 patients âgés (65) sans biothérapie en cours ni antécédents de rhumatismes inflammatoires. Une analyse des comorbidités a été réalisée pour les trois groupes et une analyses des caractéristiques rhumatologiques et thérapeutiques en rapport au rhumatisme a été réalisé pour les groupe 1 et 2. Des analyses comparatives des antécédents ont été réalisés entre les deux groupes rhumatologiques (groupe 1 et 2) et entre les deux groupes gériatriques (groupe 1 et 3). Le critère de jugement principal était la survenue des complications infectieuses, et comme critère de jugement secondaire la survenue des autres complications (soit hématologiques, soit néoplasiques, soit cardiovasculaires, soit pulmonaires). Elles sont la conséquence de réactions inflammatoires dérégulées. Les cytokines pro inflammatoires, et plus particulièrement le facteur de nécrose tumoral de type alpha (TNF-a), tiennent une place centrale dans ces phénomènes inflammatoires chroniques. Des médicaments s'opposant à leur action ont donc été mis au point. Au sein de l'arsenal thérapeutique, nous retrouvons les biothérapies ciblant le TNF-a. Apparues au début des années 2000, elles ont révolutionné la prise en charge de plusieurs pathologies et redonné de l'espoir à de nombreux patients. Infliximab, Etanercept, Adalimumab, Certolizumab, Golimumab ; les anti TNF-a sont aujourd'hui au nombre de cinq et sont devenus une classe thérapeutique incontournable. L'arsenal thérapeutique cible d'autres acteurs de l'inflammation, du type IL2, CTL4, CD20 (tocilizumab, abatacept, rituximab). Six d'entre eux, peuvent être dispensés dans les officines, ce qui requière toute l'expertise du pharmacien. Pour cela, une étude réalisée au sein du service de Rhumatologie du CHU Pierre Paul Riquet a permis de démontrer de façon significative l'acquisition des connaissances chez les patients atteints de rhumatisme inflammatoire chronique ayant suivi une séance d'éducation thérapeutique réalisée à l'hôpital sur les biothérapies. Afin de maintenir ce niveau de connaissances et de poursuivre l'éducation thérapeutique en ville, une plaquette récapitulant les messages clés sur les biothérapies a été élaborée afin d'être donnée aux patients par leurs pharmaciens officinaux.

Au cours de cette thèse, j'ai donc dressé plusieurs profils de patientes afin de comprendre ce qu'un laboratoire pharmaceutique commercialisant ces biothérapies pourrait apporter de nouveau à ces patientes.

Book excerpt: It is with great pleasure that I write this Foreword to the Proceedings of the International Conference on Behçet’s Disease which was held in Berlin in June 2002. This was the first International Conference held under the auspices of the International Society for Behçet’s Disease which was founded in 2000 in Seoul. First, I congratulate our colleagues in Berlin, led by Professor Christos Zouboulis of the Department of Dermatology at the Free University of Berlin, for having organised a most successful conference and for having compiled these proceedings so rapidly. It will be realised immediately on scanning the contents of this book that the conference was truly international with 210 participants from 26 countries, as Professor Zouboulis has noted in his preface. These included basic scientists, epidemiologists, pathologists, clinicians and, importantly, representatives from patient organisations. The latter held their own conference alongside the scientific-medical conference to mutual benefit. The combined session of patients and doctors (abstracts on pp 601 – 626) gave the opportunity for an exchange of information and fruitful discussion. The wide ranging scope of the communications is evident from the index and it was most encouraging to see their origin – from all parts of the world, from senior and junior colleagues and, from many different disciplines. Many communications may be regarded as preliminary reports of research in progress and we look forward to seeing the definitive publications in appropriate journals in due course.
