Book excerpt: Introduction : L'exposition à l'altitude est un facteur favorisant d'accident de décompression (ADD) après une plongée. Ce phénomène a essentiellement été étudié lors du voyage aérien, et peu d'études sont disponibles sur l'exposition à l'altitude par voie terrestre. Le but de ce travail est d'étudier ce phénomène sur l'île de La Réunion dans la perspective d'une action préventive au sein de la population. Méthode : Il s'agit d'une étude rétrospective portant sur les dossiers pris en charge par le service de médecine hyperbare du Groupement Hospitalier Sud Réunion de Saint Pierre. Les critères d'inclusion sont : tout accident de désaturation pris en charge par le service de médecine hyperbare du GHSR entre le 1er mai 1991 et le 31 juillet 2011 avec une exposition à une altitude supérieure ou égale à 300 mètres après la plongée. Le critère d'exclusion était l'absence d'apparition ou d'aggravation de la symptomatologie après exposition à l'altitude. Résultats et discussion : 16 cas ont été inclus et 4 exclus, ainsi 12 cas ont été retenus sur les 404 ADD répertoriés lors de cette période. 2/3 des cas sont apparus dans les 5 dernières années. L'altitude n'excédait pas 700 métres pour 66% des patients, et un autre facteur de risque d'ADD était identifié dans 2/3 des cas. L'altitude apparait plutôt comme facteur déclenchant d'ADD (2/3 cas) qu'aggravant (1/3). Limites : il s'agit d'une étude rétrospective portant sur un faible effectif. L'altitude apparait plutôt comme facteur déclenchant qu'aggravant, en notant toutefois un biais de sélection (il existe plus de personnes déclenchant un accident de décompression en altitude que des personnes montant en altitude une fois l'accident déclaré) néanmoins ces données concordent avec les données de la littérature. Conclusion : Ce travail a permis de mettre en évidence la majoration de l'incidence des ADD liés à l'altitude sur l'île de la Réunion afin de réaliser une action préventive au sein de la population.

Tests in mice show that various gases exhibit qualitative and semi-quantitative differences in the characteristics of sickness, reactions to hypoxia, and the time before the onset of symptoms. A computer, programmed for nonlinear gas transfer and other variables, running in real time can compute directly from the breathing mixture and provide a real time solution to decompression sickness under various conditions. A combined therapeutic approach, recompression and dextran (an effective lipemic clearing agent) should be capable of treating decompression sickness in humans. Other papers investigate the influence of inert gases and pressure on the central nervous system, as well as, situations in undersea and manned chamber operations. This collection can prove valuable for physiologists, biochemists, cellular biologists, and researchers involved in deep sea diving.
