Book excerpt: Un accident avec exposition au sang est un contact accidentel avec du sang ou un liquide biologique contaminé par du sang, survenant par effraction cutanée (piqûre, coupure) ou par projection sur une muqueuse ou sur une peau lésée (plaie, eczéma...) A l’occasion d’une telle exposition, trois virus (Virus de I’ Immunodéficience Humaine, Virus de l’hépatite B et Virus de l’Hépatite C) constituent l’essentiel du risque de contamination. En pratique, bien que les odontologistes soient statistiquement peu concernés (14 cas de séroconversion VIH recensés au 31 décembre 2007), l’utilisation de matériel piquant / coupant expose constamment les praticiens à un risque potentiel qu’il est utile de diminuer en utilisant de simples précautions. Compte tenu du pronostic défavorable lié à l’infection ou la coïnfection de ces virus, il apparait indispensable d’adopter une attitude préventive efficace. L’évitabilité de ces AES nécessite des formations sans cesse répétées auprès des professionnels; elle appelle un renforcement des recherches sur les matériels de sécurité et leur mise effective à disposition. Mais rien de tout cela n’ est utile si le personnel concerné n’ adhère pas activement à ces mesures. Ainsi nous avons tenté d’inclure une approche comportementale à une enquête épidémiologique réalisée auprès des étudiants en chirurgie dentaire. Nous avons mené cette enquête sur une semaine complète au Groupe Hospitalier Pitié Salpetrière. Cette enquête a deux volets : un questionnaire auprès des étudiants sur leurs connaissances, attitudes et pratiques dans la prévention des AES. Une observation in situ des pratiques (ports de gants, lunettes, masques, recapuchonnage des aiguilles, respect des circuits d’ élimination des déchets...). Nous avons analysé les résultats et comparé les pratiques observées et les connaissances.

Le risque de transmission d'éléments infectieux lors d'un AES concerne tous les types de germes présents dans le sang et les liquides biologiques. En France, c'est essentiellement le VHC, le VHB et le VIH que l'on redoute du fait de leur prévalence et de la gravité des infections engendrées. Il est donc important de garder à l'esprit que les AES ne sont pas sans danger. Pour rappel, en 2014 au SCTD, 8 trithérapies post-exposition ont été mises en place. Même si les risques de transmission sont faibles, des cas avérés de séroconversions ont été documentés en France. L'analyse des AES au SCTD et le sondage réalisé auprès des étudiants nous ont permis d'une part d'évaluer le niveau de connaissances des étudiants vis à vis des AES, et d'une autre part de mettre en avant des propositions d'amélioration au sein de la structure en matière de prévention contre ces accidents."

Face à l'ampleur de la menace, le ministère de la santé a mis en oeuvre, sous l'égide du Réseau d'alerte d'investigation et de surveillance des infections nosocomiales (Raisin), une méthode standardisée de surveillance des AES dont les établissements de santé français ont disposé de 2002 à 2016. Les chirurgiens-dentistes et les étudiants en Odontologie apparaissent comme un groupe professionnel particulièrement exposé à ce type d'accident, car leur pratique quotidienne leur impose l'utilisation d'instruments invasifs complexes dans le milieu naturellement septique que constitue la cavité buccale. La couverture de ce risque spécifique s'inscrivant pleinement dans la démarche qualité engagée au CHRU de Nancy, cette thèse se propose d'étudier les fiches de recueil issues de la surveillance nationale concernant ses services odontologiques et de contribuer à l'amélioration de la prévention des AES en établissant une nouvelle maquette de recueil des données adaptée à la pratique dentaire.
