Book excerpt: Based on sophisticated clinical descriptions and characteristic symptom patters occurring during the long-term course of psychiatric diseases, Leonhards classification requires that all the clinical features fit for a diagnosis to be made. Always using his own life-long case studies, Leonhard divided the endogenous psychoses into five main nosological groups: on the one hand, the prognostically favourable unipolar affective psychoses, bipolar affective psychoses and cycloid psychoses, and, on the other hand, the prognostically unfavourable unsystematic and systematic schizophrenia. In the meantime, the reliability and validity of this classification have been confirmed by scientific studies and early results provide guidelines for the different origins and varied therapeutic approaches for the specific disease profiles.

Book excerpt: Contradictory results in schizophrenia research are generally explained as being due to genetic heterogeneity and multiple factor heredity in relationship to manifold environmental factors. The book reports a short overview of all relevant twin studies on schizophrenic psychoses and provides data and case histories on a systematic twin study based on a polydiagnostic approach carried out by two independently working psychiatrists. In addition to the internationally applied operational diagnostic systems of DSM-III-R and ICD 10 Leonhard’s subclassification of schizophrenic psychoses was used. Up to now this sophisticated methodological approach is unique in the world. The data provide strong evidence that the spectrum of psychoses with schizophrenic and schizophrenia-like symptoms is not a continuum of diseases. At least in Leonhard’s three major groups of cycloid psychoses, unsystematic schizophrenias and systematic schizophrenias genetic, somatic and psycho-social factors play a completely different etiological role. Cycloid psychoses and systematic schizophrenias are predominantly caused by "environmental” factors. In unsystematic schizophrenias, however, genetic predisposition is the main etiological factor and environmental factors are subordinate.

Available in PDF, EPUB and Kindle. Book excerpt: In order to accurately describe and diagnose psychiatric illness, practitioners require in-depth knowledge of the signs and symptoms of behavioral disorders. Descriptive Psychopathology provides a broad review of the psychopathology of psychiatric illness, beyond the limitations of the DSM and ICD criteria. Beginning with a discussion of the background to psychiatric classification, the authors explore the problems and limitations of current diagnostic systems. The following chapters then present the principles of psychiatric examination and diagnosis, described with accompanying patient vignettes and summary tables, and related to different diagnostic concerns. A thought-provoking conclusion proposes a restructuring of psychiatric classification based on the psychopathology literature and its validating data. Written for psychiatry and neurology residents, as well as clinical psychologists, it is invaluable to anyone who accepts the responsibility for the care of patients with behavioral syndromes.

Available in PDF, EPUB and Kindle. Book excerpt: Schizophrenia may not be a single disease, but the result of a diverse set of related conditions. Modern neuroscience is beginning to reveal some of the genetic and environmental underpinnings of schizophrenia; however, an approach less well travelled is to examine the medical disorders that produce symptoms resembling schizophrenia. This book is the first major attempt to bring together the diseases that produce what has been termed 'secondary schizophrenia'. International experts from diverse backgrounds ask the questions: does this medical disorder, or drug, or condition cause psychosis? If yes, does it resemble schizophrenia? What mechanisms form the basis of this relationship? What implications does this understanding have for aetiology and treatment? The answers are a feast for clinicians and researchers of psychosis and schizophrenia. They mark the next step in trying to meet the most important challenge to modern neuroscience – understanding and conquering this most mysterious of human diseases.

Available in PDF, EPUB and Kindle. Book excerpt: Psychosis has many causes. Psychiatrists typically receive the most thorough training in its diagnosis, but the diagnosis of psychosis secondary to nonpsychiatric conditions is not often emphasized. An understanding of the underlying cause of psychosis is important for effective management. The Diagnosis of Psychosis bridges the gap between psychiatry and medicine, providing a comprehensive review of primary and secondary causes of psychosis. It covers both common and rare causes in a clinically focused guide. Useful both for teaching and reference, the text covers physical and mental state examination, describes key investigations, and summarizes the non-psychiatric features of medical conditions causing psychosis. Particularly relevant for psychiatrists and trainees in psychiatry, this volume will also assist neurologists and general physicians who encounter psychosis in their practice.

Available in PDF, EPUB and Kindle. Book excerpt: The study of catatonia in Autism Spectrum Disorders is a novel and controversial topic. Catatonia is a motor disorder characterized by stereotypy, rigidity, mutism, and posturing. These motor signs are also characteristic of autism. The interest in the relation between autism and catatonia stems from clinical observations that autistic and catatonic symptoms overlap, that some people with autism develop full-blown catatonia, and that anti-catatonic treatments bring relief in some of those patients. Catatonia in Autism Spectrum Disorders explores the question of if the two syndromes have a common pathophysiology. It also examines whether the successful treatment of catatonia be applied to patients with both autism and catatonia. The book concludes with blueprints for the assessment, treatment, and future study of catatonia in Autism Spectrum Disorders. These blueprints aim to increase early recognition and treatment of catatonia in patients with autism, show the urgency of controlled treatment trials and increased collaborative and interdisciplinary research into the co-occurrence of these two enigmatic disorders. Provides a historical perspective of the concepts of catatonia and autism, as described by old European masters like Bleuler and Kalhbaum Presents clinical-epidemiological studies that apply systematic criteria of catatonia in populations with Autism Spectrum Disorders Includes a review of the importance of psychomotor function for normal and abnormal development

Although they are included in internationally accepted diagnostic systems, our understanding of these disorders remains at a minimal level. This book is the first comprehensive overview of their clinical features, biology, course and long-term effects. The authors also address their nosological status and impact on our understanding of the continuum of psychotic and affective disorders.
