Now let us ask, What is fever Here you observe we are altogether casting aside any nominal fever. We are not speaking of scarlet fever, of typhus fever, and so on; we are taking fever in the general. I daresay you are all aware that fever has been attributed by different authors to local affections; by some to affec tion of the head; by some - the Broussaists I mean to an affection of the intestines. Certainly local diseases may produce a febrile state. We have seen that inflammation gives rise to fever; but this is not the fever of which we are now speaking. We speak now of idiopathic, or as some call it, essential fever; and there is no doubt whatever that idio pathic fever has a real existence, - that there is essential fever, - that there is fever which may kill a man without any local lesion whatever. There is not the least doubt of that. I have inspected the bodies of patients dying of typhus, and found no lesion whatever to which we could attribute the symptoms under which they laboured. Your ability to undertake this task pre supposes an acquaintance with the body in health in the most com prehensive sense; with the laws of healthy function; and with the physical signs by which you ascertain the healthy condition of the viscera of the chest and abdomen, their bulk, situation, and relation to each other. To these you refer as standards of comparison in your examination of the same organs in disease. This last is a preli minary qualification so, easy of acquirement, and, at the same time, so neglected by students, that I may be excused for pressingit upon your attention. The value of all physical signs depends on com parison with healthy standards. Alterations in the shape of one side-of the chest, dulness on percussion, absence or change of re spiratory sounds, also of the sounds of the heart in different situa tions, are all determined by comparison; first, with corresponding parts in the same individual, but ultimately, though tacitly and. Instinctively, as it were, with standards consisting of our memory' of the healthy signs. And yet'how often does some student 'press forward to the bedside, that he may listen to diseased phenomena, who has never heard the sounds of the heart or lung in health. The same rule applies to the derangements of function. To' observe these with accuracy, you must be familiarly acquainted with their healthy exercise, and, cetert's paribus, the student whose knowledge of physiology is the most perfect will excel as an oh server of disease - as a pathologist in the true meaning of the term. Book excerpt: Excerpt from The Nature of Fever: Being the Gulstonian Lectures Delivered at the Royal College of Physicians of London in March 1887In the first place it is necessary to state what I mean by fever. I need hardly say that I do not refer exclusively to the specific infective diseases grouped together as the erup tive or continued fevers. I speak of the condition of feverishness or pyrexia which is common to these diseases and to many others - the disordered inward condition of the body-heat, whose frequent and most manifest outward sign is high temperature. It is of course true that the term fever conveys to our mind a more complex notion - it implies, in general, disorder of the circulation and the respiration, of secretion and digestion, of the nervous system and the mus culat system, as well as of the body-heat. But without say ing, as some have said, that all the other disorders of function which we describe as febrile are merely consequences of the disorder of temperature, I shall not be going too far if I state at the outset that the essential fact in fever - the condition which is always present whether other of its symptoms are present or absent - is disorder of the body-heat. In using the word essential rather than primary, I wish to avoid the pre-judgment suggested by the latter. Other conditions may precede in time or in causation the essential symptom of disordered heat, and according to the confidence or the refinement of our analysis we may term one. Or another of them primary, but not until the essential symptom is present have we to do with fever. We have a like state of things inthe case of inflammation. The single word covers a whole series of processes; each process is a complex of many ele ments. It implies changes textural and changes vascular, changes nervous and changes mechanical. And in the gradual development of pathological knowledge now one and now another of these has been regarded as primary. Book excerpt: Excerpt from Observations Upon the Natural History of Epidemic Diarrhoea The causative agencies from which springs the plentiful harvest of child mortality in large cities may be conceived as a felted mass of rootlets almost inextricably intertwined. Those of poverty, bad housing, bad feeding, and neglect, may be severally recognized, but the extent of their interrelations with actual respiratory and alimentary disease - those to which the greater part of the mortality is referred - can be traced only with difficulty. It is the object Of this work to aid ln the labour of cutting away the matrix and entangling fibres and to lay bare the hidden ramifications of at least one important causative agency - epidemic diarrhoea. This affection - if we accept provisionally the more novel and generally favoured conception. As to its nature - is revealed as something very like an ordinary infectious disease, and one which permeates all classes, while the excessive mortality it gathers round itself in urban centres must be regarded as something superadded, owing to the vicious circle it forms with those baneful conditions of slum life mentioned above. On the other hand, its peculiarly intimate association with the circum stances of domestic life, from the continual faecal pollution of the interior of the household by infants and others, tends to make it more so than other affections of the kind peculiarly a class disease, and an especial scourge of dirty neighbourhoods. Dirty towns may however be saved from excessive mortality by a high percentage of breast feeding. A notable point in the interesting comparison that can be drawn between diarrhoea and typhoid fever is that, in accordance with their peculiarly opposite age-incidence curves, the marked and habitual depositing of infectious excreta within the household in the former disease may make the question of water-closet versus conservancy pan a matter of far less importance than in the latter.
