Book excerpt: Le début de la grossesse, une fois le diagnostic établi, est un moment important dans la vie d'une femme. C'est aussi le moment où s'instaure, en consultation de médecine générale, une relation particulière entre la patiente et le médecin. L'écoute du médecin et les paroles qu'il va livrer à la femme enceinte lors des premières consultations qui suivent le diagnostic de la grossesse sont primordiales. Elles vont influencer la suite du vécu et du suivi de la grossesse. Nous avons donc souhaité étudier les consultations qui se déroulent au début de la grossesse. Pour cela, nous avons réalisé une enquête auprès de femmes enceintes et de médecins généralistes par l'intermédiaire de questionnaires. Cette enquête s'est déroulée entre février et juin 2004 et nous a permis d'interroger 90 femmes enceintes ou venant d'accoucher et 35 médecins généralistes. L'exploiation des questionnaires nous a permis de soulever quatre points importants concernant le début de grossesse et le médecin généraliste. Les recommandations publiées concernant l'alcool, le tabac, les drogues, l'alimentation, le sport ou la sexualité ne sont pas toujours correctement transmises et comprises par les femmes en début de grossesse. L'anxiété est omniprésente malgrè la nature heureuse de l'avènement qui motive les consultations. La médication de la grossesse, conséquence de l'évolution de la science et de la société, est vécue de manière paradoxale par les femmes et par les médecins. Et enfin le médecin traitant tient une place importante dans le suivi de la grossesse de ses patientes. Il est présent à ce moment particulier de leur vie mais son rôle est également d'assurer une continuité des soins dans la durée et à tous les membres de la famille.

Il paraissait intéressant d’évaluer les connaissances et les gestes techniques acquis des internes de médecine d’Amiens pour le suivi de grossesse normale et la réalisation du frottis cervico-utérin. Méthode : une étude descriptive déclarative quantitative a été réalisée auprès des internes de médecine générale de la faculté d’Amiens. Les internes ont été interrogés par questionnaire sur leurs connaissances théoriques et l’acquisition des gestes permettant le suivi de grossesse normale et la réalisation du frottis. Résultats : 52 internes (38 femmes et 14 hommes) ont répondu au questionnaire. 42 internes souhaitaient inclure le suivi gynécologique dans leur pratique professionnelle. Seuls 9 internes avaient effectué un stage en gynécologie. Un semestre dédié à la gynécologie était estimé nécessaire pour 59,6% des internes. Des lacunes dans les connaissances théoriques sur le suivi de grossesse ont été relevées. Pour les gestes techniques, le stage chez le médecin généraliste était le principal lieu d’acquisition mais 36,6% des internes n’avaient jamais réalisé seul de frottis au cours de l’internat. Les internes ne se sentaient pas compétents à 38,5% pour le suivi de grossesse normale et à 34,6% pour réaliser un frottis. Discussion : un nombre plus important de stages réalisés chez le médecin généraliste permettrait à l’interne de mieux se confronter au suivi gynéco-obstétrical. Conclusion : cette étude a montré la nécessité d’améliorer la formation en gynécologie-obstétrique des internes de médecin générale.
