La Stérilisation Centrale du CHU de Poitiers prend en charge les étapes de nettoyage, recomposition, conditionnement et stérilisation des dispositifs médicaux réutilisables des blocs opératoires et des services de soins de l'ensemble de l'établissement. La maîtrise de chacune des étapes de prise en charge est nécessaire au maintien de la qualité de ce procédé. L'objectif de ce travail de thèse est de mettre en place des actions d'amélioration et de sécurisation de la prise en charge des dispositifs médicaux réutilisables à la Stérilisation Centrale du CHU de Poitiers. La réglementation appliquée à l'activité de stérilisation et aux dispositifs médicaux est abordée dans un premier chapitre, afin de mieux cerner les enjeux et choix auxquels nous avons été confrontés au cours de ce travail. Dans un deuxième chapitre, l'organisation de la Stérilisation Centrale est décrite telle qu'elle était avant le démarrage de ce travail de thèse, ceci permettant de définir les actions à mener. Dans un troisième chapitre, le développement de chacun des axes d'amélioration et de sécurisation de la prise en charge des dispositifs médicaux réutilisables est détaillé. L'axe de travail principal correspond à la mise en place de la traçabilité informatique complète de l'ensemble des compositions prises en charge par le service. Cette traçabilité a permis la mise en place d'autres axes de travail dont les consignes de prise en charge des compositions lors de l'étape de nettoyage, une validation pharmaceutique des cycles des laveurs-désinfecteurs et la réalisation de statistiques d'activité sur le matériel retraité au sein de la Stérilisation. L'amélioration du contrôle du conditionnement des conteneurs s'inscrit également dans l'objectif global de ce travail de thèse. A ce jour, toutes les actions développées et décrites dans ce mémoire sont appliquées en routine par l'ensemble de l'équipe. Le travail réalisé, bien que pouvant être constamment amélioré, constitue une réelle avancée en termes d'amélioration et de sécurisation de la prise en charge des dispositifs médicaux réutilisables à la Stérilisation Centrale du CHU de Poitiers.

The purpose of the second edition is to expand and update the practical information in the original Blue Book. The new Blue Book is designed to continue to be a source of impartial health-care information and guidance on safe waste-management practices. The editors' intention has been to keep the best of the original publication and supplement it with the latest relevant information. The audience for the Blue Book has expanded. Initially, the publication was intended for those directly involved in the creation and handling of health-care wastes: medical staff, health-care facility directors, ancillary health workers, infection-control officers and waste workers. A wider range of people and organizations now have an active interest in the safe management of health-care wastes: regulators, policy-makers, development organizations, voluntary groups, environmental bodies, environmental health practitioners, advisers, researchers and students.
