Book excerpt: L’objectif de cette étude était d’étudier le rôle du support social sur le risque d’entrée en institution et de mortalité avec un recul de six mois, chez des personnes âgées de 75 ans ou plus hospitalisées en urgence dans un service de médecine de court séjour. L’étude a été réalisée à partir de la cohorte française (SAFES) composée de sujets âgés de 75 ans ou plus admis dans un service de court séjour de médecine. Étaient recueillies des données sociodémographiques et d’évaluation gériatrique standardisée. Les événements analysés par une régression de Cox étaient l’institutionnalisation et la mortalité dans les six mois ayant suivi l’admission aux urgences. Sur 1607 sujets éligibles, 1306 ont été définitivement inclus la cohorte. Leur âge variait entre 75 ans et 103 ans avec une moyenne de 85 ± 6 ans. La plupart étaient des femmes (65%, n= 845). Leur nombre moyen d’enfants étaient de 2 ± 2. Après six mois de suivi, 184 sujets (17,6%) étaient institutionnalisés et 319 (24,4%) étaient décédés. Parmi ces décès, 50 (15,7%) étaient survenus en institution. En analyse multifactorielle, après ajustement sur le niveau de comorbidité et le centre d’investigation, les facteurs qui augmentaient significativement le risque d’entrée en institution étaient : un âge supérieur à 85 ans (RR= 2,89 ; IC 95 %= 2,04 – 4,10), et la présence d’un aidant principal (RR= 1,81 ; IC 95 %= 1,16 – 2,83). En revanche, le nombre d’enfants étaient inversement lié au risque d’entrée en institution (RR= 0,81 ; IC 95 %= 0,73 – 0,91). Chez les sujets ayant un aidant principal, le fardeau de l’aidant ajusté sur les autres facteurs, était un facteur de risque d’entrée en institution (RR = 2,43 ; IC 95 % = 1,51–3,90). Concernant la mortalité, un âge supérieur à 85 ans augmentait significativement le risque de décès dans les six mois (RR = 1,65 ; IC 95 % = 1,25 – 2,19), tandis que et le fait de vivre seul réduisait ce risque (RR = 0,68 ; IC 95 % = 0,50 – 0,91). Contrairement à l’entrée en institution, l’existence d’un fardeau de l’aidant ne semblait pas influencer de façon indépendante le de risque de mortalité à six mois. Les résultats de cette étude sont en faveur de la mise en place d’une prise en charge précoce gérontologique pluridisciplinaire (gériatre, assistante sociale, psychologue...) L’équipe mobile gériatrique pourrait être « l’outil » idéal.
