La région Grand Est et la Meuse sont particulièrement touchées par la toxicomanie aux opiacés avec une forte présence d'héroïne circulant sur le territoire. La position géographique de la Meuse participe à la disponibilité du produit et à la facilité de se le procurer pour les habitants des villes et des petits villages meusiens. Face aux consommations observées, de nombreux patients bénéficient d'un traitement de substitution aux opiacés délivré sur prescription en pharmacie. Le pharmacien détient ainsi un rôle important dans l'encadrement des patients toxicomanes et veille au bon déroulement du traitement. Il assure le respect des délivrances, l'approvisionnement et la gestion des stocks des médicaments de substitution. Il prend part également à la coopération interprofessionnelle avec les équipes soignantes en addictologie. Le pharmacien participe aux actions de prévention en délivrant des Stéribox et en participant à des programmes d'échange de seringues. Une enquête a été réalisée auprès des pharmaciens meusiens et interroge sur la prise en charge des patients sous substitution à l'officine. L'enquête apporte une description de la répartition des patients parmi les officines, des relations pharmaciens/patients et s'intéresse au coté pratique lors de la délivrance de médicaments de substitution. Elle donne un aperçu du suivi des toxicomanes par les officines au cours de l'année 2019 en Meuse. Les résultats de l'enquête ont permis de réaliser des outils d'aide à la prise en charge à disposition des pharmaciens tels que : un dossier patient, des fiches mémos pharmacien et médecin, des fiches patient et des procédures d'aide à la délivrance.

L'état des lieux effectué de l'accès aux différents TSO en France et dans ses différentes régions le prouve.Grâce à une collaboration avec patients et pharmaciens, nous avons pu faire un constat plus précis sur la diffusion des TSO et la prise en charge de l'usager de drogues dans la communauté de commune du Lunévillois.L'enquêtemenée sur le Lunévillois a permisde montrer que le nombre de patients sous TSO dans la commune est très supérieur à la moyenne nationale et que l'importance du phénomène nécessite la miseen place de certains outils pour mieux répondre à la politique de réduction des risques.Pour cela des fiches conseils ont été mises en place dans le but d'aider le pharmacien d'officine à une meilleure prise en chargede l'usager de drogues au quotidien.

Leur usage non médical, abusif ou sans supervision médicale peut conduire à la dépendance, voire à une surdose pouvant entrainer la mort. On estime à plus de 35 millions dans le monde le nombre de personnes dépendantes aux Opiacés. Environ 210 000 décès sont enregistrés et dont presque la moitié est liée à une surdose d'Opioïdes. Ce constat mondial est plus qu'alarmant voire prend le sens d'une crise sanitaire sans précédent aux États-Unis. La France n'est pas dans une situation comparable mais l'augmentation rapide du nombre d'intoxications, d'hospitalisations et de décès tend à devenir un véritable enjeu de Santé Publique. Une molécule antidote existe : la Naloxone. Après une première mise à disposition aux usagers de drogues de kits de naloxone à emporter à domicile aux États-Unis à la fin des années 1990, de nombreux pays, notamment européens, ont aussi lancé leurs programmes de distribution de Naloxone. La molécule a été disponible en France en 2015 dans un premier temps avec la spécialité Nalscue®, puis avec le Prénoxad®. Pour ce dernier, il s'agit d'un dispositif prêt à l'emploi destiné à l'administration sous cutanée de Naloxone, disponible désormais en Officine. En effet, le métier de pharmacien est en perpétuelle évolution, et ainsi au-delà de la dispensation du médicament, il devient un véritable interlocuteur de confiance auprès de ses patients dépendants. Dans un contexte, où le nombre de décès lié aux Opioïdes est en augmentation, les avancées de la mise à disposition pour tous de la Naloxone est plus que salutaire et promet un avenir certain pour la prise en charge des overdoses.

Elle est un phénomène insidieux et souvent imprévu, qui une fois installée chez sa victime demandera un grand contrôle psychique et physique pour s'en défaire. Malheureusement, l'addiction n'est pas seulement induite par des drogues « dures » ou des comportements tels que l'addiction aux jeux d'argent, au sexe... des médicaments peuvent être aussi en cause. C'est le cas des antalgiques opioïdes utilisés dans le cadre de traitement de la douleur, qui, à ce jour, déclenchent un problème sanitaire de taille en France. Cette thèse a pour objet de présenter ces médicaments, de renseigner leur place au sein du traitement de la douleur, ainsi que d'exposer leurs grands effets indésirables addictologiques que sont le mésusage, la dépendance, le syndrome de sevrage et le surdosage aux opioïdes de prescription. Elle indique également quelles pourraient être les mesures futures à adopter pour éviter leur survenue. Un autre aspect de ce travail est de mettre en lumière le rôle du pharmacien d'officine dans la prise en charge thérapeutique des patients consommant des antalgiques opioïdes afin d'éviter la survenue des effets indésirables, cités précédemment, et de minimiser la progression de la crise sanitaire aux opioïdes qui en découle.

Opioid overdose is easily reversed with the opioid antidote naloxone and with basic life support. Such care is generally only available in medical settings, however. These guidelines recommend that people who are likely to witness an opioid overdose, including people who use opioids, and their family and friends should be given access to naloxone and training in its use so that they can respond to opioid overdose in an emergency if a medical response is not available. Naloxone can be injected or administered intra-nasally and has minimal effects in people who have not used opioids. While naloxone administered by bystanders is a potentially life-saving emergency interim response to opioid overdose, it should not be seen as a replacement for comprehensive medical care.
