Le repérage des patients en médecine générale est la première étape de la prise en charge. L'objectif principal est d'étudier le repérage des sujets à risque de fragilité, de 65 ans et plus, non dépendants, consultant en cabinet de Médecine Générale, selon le questionnaire de repérage recommandé par la Haute Autorité de Santé. L'objectif secondaire est d'étudier les caractéristiques des patients repérés comme à risque de fragilité en ambulatoire. Nous avons conduit une étude observationnelle prospective chez des sujets âgés de 65 ans et plus, ambulatoires et non dépendants, consultant dans 6 cabinets de médecine générale en Ile-de-France, de Janvier 2013 à Décembre 2014. Nous avons soumis à ces patients le questionnaire de repérage recommandé par la HAS. Cent-six patients ont bénéficié du repérage de la Fragilité en médecine ambulatoire. La moyenne d'âge était de 74.3 ans (6,8). 52,8% étaient des femmes (n= 56). 45,3 % des patients étaient repérés à risque de fragilité et 54,7% non à risque de fragilité. Le critère de risque de fragilité le plus fréquent était de vivre seul (27,3%). Il n'existe cependant que peu de données sur les retombées cliniques d'une telle évaluation et sur les bénéfices attendus. Nous avons réalisé une étude observationnelle rétrospective sur 1 an, afin de décrire une population âgée de plus de 65 ans adressée en HDJ gériatrique et repérée comme fragile, d'analyser l'évolution à 1 an de cette population, et de tenter d'établir un lien entre dépistage de la fragilité et évolution clinique. 50 patients fragiles ont été inclus, et 43 ont pu être réévalués. L'évolution de cette cohorte fragile retrouve à 1 an la survenue de nombreux évènements indésirables, parmi lesquels perte d'autonomie, chutes et hospitalisations. L'évaluation réalisée permet la mise en application efficace de préconisations ciblées en ambulatoire, en lien avec le médecin traitant. La mise en place de préconisations nutritionnelles modifie de manière significative l'état nutritionnel biologique des patients (p=0,0001). Les préconisations motrices sont insuffisamment prescrites. Il existe une majoration significative du nombre d'aides professionnelles (p=0,0003). L'impact de l'évaluation de la fragilité sur l'évolution clinique reste peu documenté, mais un bon suivi des préconisations semble corrélé à une diminution du risque de survenue d'évènements indésirables. Des études à grande échelle sont souhaitables pour préciser les modalités de suivi permettant d'améliorer significativement le pronostic des patients âgés fragiles.
