L'acteur passif qu'était le malade est devenu un interlocuteur actif, revendiquant sa qualité juridique de consommateur. De plus, le colloque, initialement singulier, est devenu plural. Entre le médecin et le malade, ou patient, divers éléments s'interposent maintenant (chaîne d'intervenants multiples, contexte économique, législateur, pouvoirs publics, source jurisprudentielle) et ont fait irruption dans la relation. Si les deux acteurs initiaux se retrouvent finalement face à face, d'autres partenaires vont fortement influer sur les rapports entre le médecin et le malade. Il appartiendra cependant au praticien de prendre seul la décision qui engage sa responsabilité vis à vis de son patient. Mais cette décision l'engage également, par de multiples liens, envers les autres éléments du système. Il demeure "libre" de décider, mais ses choix doivent s'inscrire dans une épure normative de plus en plus contraignante. De plus, le résultat de cette décision n'est plus qu'une proposition de soins présentée au patient qui seul, mais après une parfaite information et en collaboration avec le professionnel, prendra in fine les résolutions qu'il jugera utile pour sa santé. Ainsi est institutionnalisé un régime de codécision qui vise à remplacer la relation paternaliste si décriée. Cette nouvelle disposition, d'origine législative, permet de penser que le patient, devenu participant actif de la relation médicale, prend à son compte une partie de la responsabilité, celle du risque encouru, dont il a été dûment informé préalablement et qu'il a accepté en toute connaissances de cause.

The volume considers a full range of countries on every inhabited continent, including: Africa, Asia, Australia, Central and South America, Europe, the Middle East, and North America. Topics include government agendas such as nationalism and nation building; agendas of powerful, associated professions (e.g., medicine, law); theological and political agendas such as 'culture wars'; agendas of entrepreneurial economies of profit; and other cultural and ideological agendas consciously or unconsciously advanced or contested by bioethics work in particular countries based on their unique history, politics and culture.
