L'incidence de violence conjugale reste sous-estimée. Les femmes victimes ont peur d'en parler. Les médecins généralistes ne posent pas la question. L'objectif de ce travail est d'évaluer l'impact d'un dépistage systématique de la violence conjugale envers les femmes en cabinet de médecine générale. Méthodes : un premier questionnaire a été envoyé à 174 médecins généralistes installés dans le département des Alpes-de-Haute-Provence. Les médecins ayant répondu au premier questionnaire ont été recontactées pour réaliser un dépistage systématique des violences conjugales à l'aide du questionnaire WAST pendant 5 jours de consultation. Les données de cette étude quantitative observationnelle ont fait l'objet d'une analyse descriptive. Résultats : onze médecins ont participé au dépistage systématique. Parmi les 263 femmes interrogées pendant 5 jours, 14 ont été déclarées par le médecin généraliste comme victimes de violence conjugale (5,32%). Parmi les femmes victimes, 85 % subissaient des violences psychologiques et 50 % des violences sexuelles. Les difficultés le plus fréquemment éprouvées pour réaliser le WAST de manière systématique étaient sa réalisation chez une patientèle connue de longue date ou de façon personnelle et la peur d'une intrusion dans la vie privée. Plus de la moitié des médecins (54,5%) pensaient qu'un dépistage par signes d'alarme restait suffisant. Discussion : un dépistage systématique de la violence conjugale permet de dévoiler un plus grand nombre de femmes victimes (5,32%) qu'un dépistage passif (0,84%). Cependant, les médecins sont réticents à dépister systématiquement. Les principaux obstacles observés sont les préjugés et barrières personnelles des médecins. Il est nécessaire de briser le tabou sur le sujet des violences conjugales pour bien repérer et prendre en charge les femmes victimes.

Matériel et méthode : une méthode Delphi a été utilisée pour définir par consensus le contenu de cet outil. Un panel de 16 experts pluri-professionnels a été sollicité pour répondre aux différentes rondes. En fonction des résultats et des commentaires des experts, les items ont été modifiés puis retenus. Résultats : un premier tour de mise au point a aidé à l'identification des items importants sur le sujet en fonction de la littérature. La première ronde a permis de retenir 32 items. Les autres items ont été modifiés et validés lors de la seconde ronde. Le consensus des experts a permis de structurer un guide en 10 étapes allant du dépistage systématique, à la rédaction d'un certificat, à l'évaluation du danger jusqu'à l'orientation des patientes. Conclusion : le contenu de l'outil d'aide au dépistage et à l'orientation des femmes victimes de violences conjugales a été obtenu de manière pluri-professionnelle et consensuelle. Ce guide structuré en 10 étapes constitue une aide pour les médecins généralistes afin de mieux repérer et intervenir auprès de ces patientes.

Le médecin généraliste est le premier intermédiaire professionnel des femmes victimes. Depuis 2019, l'HAS recommande le dépistage systématique des patients devant une évidente sous-estimation des victimes. Objectif : évaluer la fréquence des consultations pour violences conjugales en médecine générale et les modalités de dépistage. Évaluer la connaissance des outils médicaux d'aide à la protection des victimes disponibles et leur application en pratique. Matériels et méthodes : par une étude quantitative observationnelle transversale, via un questionnaire transmis par voie électronique, nous avons étudié les caractéristiques sociodémographiques des médecins généralistes libéraux des Bouches du Rhône, les modalités de dépistage en cabinet, les freins au dépistage et la fréquence des consultations. Nous avons également étudié les connaissances des outils d'aide aux victimes de violences conjugales et les possibles améliorations de communication. Résultats : 82 médecins généralistes ont été inclus, la population la plus représentée étaient les femmes médecins (63.4%) avec une expérience professionnelle ≤ 5 ans ayant une activité libérale en cabinet de groupe. 91.5% des répondants ne dépistaient pas de manière systématique les violences conjugales. Sur les 12 derniers mois, le nombre médian de cas était de 1 cas par médecin. 58.6% des répondants étaient les premiers intermédiaires professionnels. La majorité des médecins se sentait légitime dans ce rôle (86%). Le facteur principal limitant au dépistage systématique était le manque de formation (47.6%). L'outil d'aide aux victimes le plus utilisé était le certificat médical initial (68%) alors que l'ordonnance de protection n'était connue que par 30% des répondants, en majorité ayant été formés aux violences conjugales. Conclusion : l'instauration de formations dès le premier cycle des études médicales, l'amélioration du travail en réseau et une meilleure communication via les grandes instances médicales amélioreraient le dépistage systématique des violences conjugales et l'utilisation des outils d'aide aux victimes par les médecins généralistes.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured?
