Elle est accessible à un traitement efficace s'il est réalisé dans les premières années de vie, avant la fin de la période sensible du développement visuel. L'amblyopie est essentiellement liée à des troubles réfractifs, en majorité hypermétropie, et strabisme. L'examen de référence pour la recherche d'une amétropie est une mesure objective de la réfraction sous cycloplégie, par un opthalmologiste. En raison d'un nombre insuffisant et décroissant d'ophtalmologistes, il n'est pas possible de proposer de manière systématique cet examen à chaque enfant. Des techniques de photo-vidéoréfraction permettent de rechercher les amétropies sans instillation de collyre cycloplégiant. L'analyse de la littérature révèle que ces appareils sont performants pour la détection des troubles réfractifs significatifs, au prix d'une sensibilité moindre pour l'hypermétropie. Afin de solutionner ce problème organisationnel, un programme de dépistage visuel pourrait instaurer une consultation par un orthoptiste à l'âge de 3 ans, avec une mesure de l'acuité visuelle de manière standardisée, photo-vidéoréfraction et examen sous écran. Ce protocole est applicable à l'ensemble de la population générale d'enfants ne présentant pas de signes d'appels ni de facteurs de risque.

La Protection Maternelle et Infantile (PMI) est au cœur de ce dépistage, par la réalisation des bilans de santé dans les écoles maternelles. L'objectif de notre étude était de décrire l'évolution sur 10 ans des résultats du dépistage des troubles visuels pratiqué par la PMI dans les écoles maternelles des Bouches-du-Rhône ; ainsi que de décrire les éventuelles disparités locales au sein du département. Il s'agit d'une étude de cohorte rétrospective, descriptive, quantitative. L'étude a porté sur tous les enfants (n=195 774) des Bouches-du-Rhône ayant bénéficié d'un bilan de santé avec dépistage des troubles visuels en moyenne section de maternelle, durant les années scolaires 2005-2006 à 2013-2014. Une étude plus détaillée au niveau local a été réalisée sur l'année scolaire 2014-2015. Cette étude révèle une bonne couverture du dépistage (87% des enfants ont bénéficié du dépistage), un taux d'orientation vers l'ophtalmologiste de 19% en moyenne avec de fortes disparités territoriales (taux variant de 11 à 33% selon l'année scolaire et le territoire), un taux de retours sur l'orientation avoisinant les 50%. L'anomalie dépistée a été confirmée par le spécialiste dans près de 90% des cas. Le protocole actuel de dépistage des troubles visuels réalisé par la PMI des Bouches-du-Rhône permet ainsi de détecter avec efficacité les troubles visuels (anomalie de l'acuité visuelle et trouble de la vision binoculaire). La confirmation par l'ophtalmologiste de l'anomalie dépistée permet une prise en charge précoce, à un stade encore accessible à une thérapeutique efficace.
