Le cerveau a développé des mécanismes spécialisés pour la décoder différents de ceux du langage, ce qui explique que l'aphasie ne s'accompagne pas nécessairement d'une amusie. Contrairement à la parole, la musique active les deux hémisphères cérébraux que ce soit pour sa perception, sa mémorisation, sa lecture, son écriture ou son exécution instrumentale ou vocale. Cela explique le cas de Maurice Ravel qui, atteint d'une lésion de l'hémisphère gauche pouvait composer, jouer et même accorder son piano alors qu'aphasique, il était incapable de lire ou écrire une seule note. Mais le cerveau est un système dynamique basé sur des modifications synaptiques qui lui permettent de s'adapter aux exigences musicales. Ainsi le cortex du musicien diffère de celui du non musicien que ce soit dans sa spécialisation hémisphérique, ses modifications anatomiques touchant le cortex auditif, sensitivo moteur, le corps calleux ou le cervelet mais aussi sans sa réorganisation cytoarchitecturale. Cependant, le cerveau musical reste encore mystérieux puisque de nombreux facteurs, difficiles à prendre en compte, influencent la localisation exacte du traitement des différentes composantes musicales. De plus, l'imagerie fonctionnelle de plus en plus perfectionnée peut paraître encore grossière par rapport à la finesse des structures cérébrales et la vitesse de conduction de l'influx nerveux.
