Book excerpt: This atlas describes and illustrates in detail the surgical procedures employed in laparoscopic anatomical liver resection, according to tumor location. The surgical approach and techniques are explained for each type of laparoscopic liver resection based on the involved anatomical segment, section, and hemi-liver. Attention is also drawn to potential complications and the means of preventing and managing them. Beyond anatomical liver resection, guidance is provided on non-anatomical resections that do not pay regard to segmental, sectional, or lobar anatomy. The coverage is completed by the inclusion of useful information on instruments and energy devices as well as the basic skills required for laparoscopic liver resection. In presenting a clear practical approach to each laparoscopic anatomical major and minor resection, the Color Atlas of Laparoscopic Liver Resection will be an invaluable guide for trainees. In addition, it will equip more experienced surgeons with much useful information that will enable them to further improve their technique.

It includes all aspects of the surgical anatomy of the liver, preoperative management of patients, surgical techniques, and intraoperative key points to prevent postoperative complications. The first three chapters provide a general introduction to the clinical anatomy of the liver, preoperative management of patients with hepatocellular carcinoma, basic techniques for hepatic resection using the Glissonean approach, and the application of dye staining in anatomic hepatic resection. Subsequent chapters present the technical details of anatomical segmentectomy (Couinaud’s classification), sectionectomy and hemi-hepatectomy for hepatocellular carcinoma using the modified suprahilar Glissonean approach. All of these hepatectomies can be performed using simple and easily available surgical instruments. In addition, it discusses precise transection of the deepest hepatic parenchyma guided by methylene blue staining. It is a useful and timely reference for hepatobiliary surgeons, clinical staff, and medical students.

Now this large amount of experience will be made universally available through this book. After a concise description of the main tools and technology necessary to carry out a safe laparoscopic and/or robot-assisted liver surgery, details of operative techniques will be illustrated. Each chapter will consist of an introduction, a summary of the indications and contraindications of specific operative procedures and a detailed description of each surgical procedure, including the potential complications of both the surgery and the postoperative care. A special effort will be done to provide informative and accurate illustrations and pictures, that will be completed by the DVD containing the movies illustrating the technical procedures performed by the leading European liver surgeons.

Available in PDF, EPUB and Kindle. Book excerpt: This book provides a comprehensive guide to the treatment of small hepatocellular carcinoma (sHCC) using a minimally invasive technique: radiofrequency ablation (RFA). RFA has emerged as a new treatment modality and become the main modality of locoregional therapy. Extensive clinical research indicates that RFA is as effective as surgical resection for sHCC, and it has the advantage of being less invasive. However, the outcomes after RFA are largely dependent on the operators’ experience- known as the “learning curve”. This book presents the characteristics of sHCC and discusses why sHCC is the best candidate for RFA. Then it introduces all the commercially available RFA systems, and their working principles, advantages, disadvantages and so on. It goes on to demonstrate how to perform RFA under the guidance of ultrasound, CT, laparoscopy, or during open operation. Finally, it discusses the radiologic assessment and follow‐up after RFA, as well as adjuvant therapies and clinical trials on RFA. The authors are experts from the fields of pathology, radiology, surgery, and gastroenterology, as well as manufacturers. With this book, readers gain have a clear idea of when and how to do RFA. It aims to standardize and generalize the procedure of RFA, which will be very helpful in improving the outcome of RFA for sHCC.

Book excerpt: Aim of this book is to present the main minimally invasive approaches to the Glissonean pedicles. This kind of technique for hepatectomy has been gaining interest in recent years and may represent the ideal method to achieve anatomical liver resections. Currently the most performed procedure for liver surgery is still the classic intrafascial approach in which, after entering the Glissonean sheath, the portal triad elements are singularly dissected and isolated. However, this is a quite challenging technique to pursue in a minimally invasive setting. Nowadays the laparoscopic and robotic approach for liver resections are increasing worldwide and the intra or extra hepatic Glissonean approach, in which the portal triad elements are dissected and isolated “en bloc” without entering the Glissonean sheath, can facilitate the surgical technique. Furthermore this strategy allows better identification of the ischemic area and could decrease blood loss during dissection, as the liver inflow is kept under control. This book includes a step-by-step description of each pedicle isolation and section, with original laparoscopic and robotic pictures and videos. HPB surgeons who are interested in this technique may find these pages, written in cooperation with some of the most prominent worldwide experts, to be highly informative and valuable.

This book was released on 2016-09-30 with total page 5 pages. Available in PDF, EPUB and Kindle. Book excerpt: Hepatocellular carcinoma (HCC) is a common malignant tumor and many cases occur in patients with liver cirrhosis. Although liver transplantation is the most effective treatment option, hepatectomy is still the first curative treatment option because liver transplantation is limited by the donors and high cost. In recent years, laparoscopic liver resection (LLR) has increasingly been performed in patients with liver cirrhosis, and has several advantages over open liver resection. Besides less pain and shorter hospital stay, LLR in patients with liver cirrhosis is also associated with lower incidences of postoperative liver failure and ascites because of greater preservation of collateral veins and less liver manipulation. With increasing experience, LLR for HCC located in segments 7 or 8 is now feasible, and anatomic LLR could be performed in patients with cirrhosis. Many comparative studies have shown that LLR is better than open liver resection in patients with liver cirrhosis in terms of a lower incidence of postoperative liver failure and similar patient survival. In conclusion, LLR is a promising treatment modality for HCC in patients with liver cirrhosis.
