Environ 50% des aînés (65 ans et plus) rapportent des difficultés de sommeil qui sont, à leur tour, associées au déclin cognitif tant subjectif qu’objectif. Sachant cela, il est pertinent de s’intéresser aux façons d’améliorer le sommeil dans cette population afin d’arrêter ou du moins ralentir le déclin cognitif et ainsi préserver la qualité de vie de ceux qui en souffrent. Les traitements pharmacologiques étant associés à des effets secondaires et à des risques liés à une utilisation à long terme, d’autres alternatives de traitement non pharmacologiques doivent être investiguées. L’objectif de cette étude pilote était donc de comparer l’effet de deux programmes d’exercices physiques sur le sommeil subjectif et objectif de personnes âgées souffrant d’un trouble cognitif léger (TCL). Douze participants de 60 ans et plus souffrant d’un TCL rapportant des difficultés de sommeil ont été assignés à un des deux groupes expérimentaux de 12 semaines d’exercices physiques en aérobie ou en résistance. La polysomnographie (PSG) pendant deux nuits à la maison a été effectuée et des agendas de sommeil ont été complétés avant et après l’intervention d’exercices physiques afin de mesurer la qualité et la quantité objectives et subjectives du sommeil des participants. Les comparaisons pré et post traitement ont révélées une diminution significative du nombre d’éveils durant la nuit dans les deux groupes. Les résultats étaient non significatifs pour les autres variables de sommeil. Toutefois, l’exploration visuelle des moyennes effectuée conjointement aux analyses de puissance suggère des améliorations sur plusieurs paramètres de sommeil subjectifs et objectifs, et ce, dans les deux groupes. Enfin, la participation à un programme d’exercices physiques a un effet significatif sur la qualité du sommeil des aînés souffrant d’un TCL. Les résultats ont été obtenus auprès d’un échantillon de petite taille. Il semble donc impératif de répliquer cette étude auprès d’un plus grand échantillon. La pratique d’exercices physiques pourrait être une approche non pharmacologique potentielle pour l’amélioration de la qualité du sommeil des personnes âgées souffrant d’un TCL.

Se situant sur un continuum allant du vieillissement normal à la démence, ils se présentent sous différentes formes, dont le trouble cognitif léger et le trouble cognitif sans démence. Ces troubles représentent souvent une étape transitoire entre le vieillissement normal et pathologique, et en ce sens, sont reconnus comme pouvant être des phases préclinique ou prodromique de la démence. Par ailleurs, ces syndromes sont souvent accompagnés de symptômes psychologiques tels que les perturbations du sommeil. Ce problème se révèle être particulièrement important à considérer, d'une part puisque les perturbations du sommeil peuvent être des manifestations précoces d'un déclin cognitif ou d'une démence, et d'autre part puisqu'elles peuvent avoir un effet délétère sur la cognition. Depuis les dernières années, plusieurs chercheurs se sont penchés sur la relation prédictive entre les perturbations du sommeil et le déclin cognitif chez les personnes âgées. Toutefois, les études procurent des résultats mitigés, faisant en sorte qu'aucun consensus n'est aujourd'hui établi quant à cette relation. L'objectif principal de ce mémoire doctoral est donc d'examiner systématiquement la littérature afin d'évaluer si les perturbations subjectives et objectives du sommeil peuvent prédire le déclin cognitif chez les personnes âgées de 50 ans et plus présentant ou non, au départ, un déclin cognitif associé au vieillissement. Les perturbations du sommeil ciblées se rapportent : a) à la durée du sommeil; b) à la qualité du sommeil; c) à l'architecture du sommeil; d) aux rythmes circadiens ; e) à l'insomnie ; et f) aux apnées du sommeil. Les bases de données scientifiques Medline, PsycInfo, Embase, Cochrane Library, Proquest Dissertations & Theses et Bielefeld Academic Search Engine ont été consultées, pour une période allant jusqu'en juin 2017. La revue systématique présente une méthodologie de recherche rigoureuse, considérant que 1) elle est réalisée selon la méthodologie du groupe Cochrane; 2) seules les études longitudinales avec des résultats permettant d'évaluer le risque prédictif des perturbations du sommeil concernant un futur déclin cognitif - odds ratio (OR), hazard ratio (HR) ou relative risk (RR) - ont été sélectionnées ; et que 3) seuls les résultats provenant des modèles statistiques ajustés pour le plus grand nombre de covariables ont été retenus. Les résultats de la revue systématique indiquent que les durées subjectives extrêmes du sommeil (courte et longue durée), qu'une faible efficacité du sommeil, que les apnées du sommeil, qu'une modification de l'architecture du sommeil et qu'une altération des rythmes circadiens sont associés à une augmentation du risque de déclin cognitif chez les personnes âgées. Bien que ces résultats ne permettent pas de parler de causalité, mais plutôt d'association, les hypothèses explicatives concernant les liens entre les perturbations du sommeil et le déclin cognitif pointent vers une relation causale bidirectionnelle entre ces deux variables. La reconnaissance des perturbations du sommeil comme marqueur précoce du déclin cognitif pourrait permettre une identification hâtive et peu couteuse des individus à risque d'évoluer vers un trouble cognitif ou une démence. De même, le traitement des perturbations du sommeil apparaît nécessaire dans la prévention du déclin cognitif et dans sa prise en charge une fois le déclin amorcé. Cela revêt une grande importance, sachant qu'une intervention clinique précoce ralentirait la progression du déclin intellectuel et fonctionnel des individus présentant un déficit cognitif.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? Structural and/or functional judgments included are organized on the OSMSE test form under categories of: lips, tongue, jaw, teeth, hard palate, soft palate, pharynx, breathing, and diadochokinesis. Results of a preliminary study with normal speaking subjects indicated that the OSMSE had satisfactory intra- and inter-examiner reliability after minimal training.
