Book excerpt: Les étudiants en situation de handicap psychique éprouvent des difficultés à mener à terme leur formation professionnelle, d'autant plus que les troubles psychiques apparaissent généralement entre 15 et 25 ans. Or, l'accès à un diplôme professionnalisant, via l'accès et le maintien d'une formation, est un indicateur d'une meilleure insertion professionnelle. Réfléchir sur les déterminants de l'accès au diplôme professionnalisant constitue un enjeu crucial en réhabilitation orientée rétablissement. Une revue systématique de la littérature a permis de mettre en évidence les déterminants des difficultés d'accès au diplôme professionnalisant, ainsi que les dispositifs existants. Cette revue a suivi au maximum les critères PRISMA et a inclus 24 articles. Les déterminants sont classés en trois catégories : les déterminants liés au contexte de formation, ceux qui dépendent de l'individu et ceux qui sont liés à la société. Plusieurs dispositifs existent afin de pallier ces facteurs : notamment, au niveau universitaire via des aménagements pédagogiques et des structures dédiées, et au niveau communautaire via des dispositifs de supports à la formation (« supported education »). Ces dispositifs restent balbutiants du fait de leur faible nombre, de leur manque de spécificité dans le ciblage des déterminants en jeu, et du manque de moyens qui leur sont en général alloués. Il s'agit avant tout d'un enjeu éthique, celui de l'accès à un projet de vie, qu'il soit éducationnel ou professionnel, pour tout individu, quel qu'il soit. Et cela ne peut se faire sans le changement de regard sur le handicap psychique, c'est-à-dire en luttant contre la stigmatisation et l'auto-stigmatisation.

Book excerpt: Introduction : selon l'Organisation Mondiale de la Santé (OMS), cinq des dix pathologies les plus préoccupantes du XXIe siècle sont des maladies psychiques : schizophrénies, troubles bipolaires, addictions, dépressions et troubles obsessionnels compulsifs. Depuis les 20 dernières années on observe des avancées concernant la question du handicap psychique et du travail. Quels sont les freins persistants et les leviers à l'intégration des Personnes en Situation de Handicap Psychique (PSHP) en entreprise en milieu ordinaire. Sujets et Méthodes : une revue narrative de la littérature a été menée de juin 2020 à décembre 2021. Les recherches bibliographiques se sont concentrées sur les publications de quatre bases de données et sur deux ouvrages traitant du handicap psychique et du travail. Résultats : l'emploi est un facteur de rétablissement pour les personnes en situation de handicap psychique. L'emploi accompagné est un dispositif permettant d'accompagner la PSHP et l'entreprise dans l'accès et le maintien en emploi. Les études montrent qu'il augmente les chances de se maintenir en emploi par rapport aux autres méthodes. Il se développe en France mais reste balbutiant et réservé aux situations les plus complexes alors que la remise précoce au travail est préconisée. Conclusion : le système de l'entreprise et le système médico administratif qui nous gouverne génèrent des obstacles à la fluidité du parcours d'accompagnement des personnes en situation de handicap psychique. Un accompagnement des entreprises, l'accès à la formation et à la sensibilisation du personnel de ressources humaines et des encadrants pour lutter contre la stigmatisation semblent indispensables pour donner l'opportunité aux personnes en situation de handicap psychique de mieux s'intégrer dans le monde de l'entreprise. La formation et le travail en réseau des professionnels de santé semblent également nécessaires. Le médecin du travail est un acteur clé du maintien en emploi. Il doit, avec les services de santé au travail se saisir de cette problématique et être inclus dans les réflexions autour de l'accompagnement de cette population.
