Book excerpt: Franco Basaglia (1924-1980) was an Italian psychiatrist and activist who proposed the dismantling of psychiatric hospitals and pioneered new ideas about mental health and its treatment. Basaglia was also one of the principal proponents of Italy's Law 180, which effectively closed down large mental hospitals in Italy. His ideas and his disciples have had a decisive influence in the move away from institutional care in many parts of the world, particularly in continental Europe and South America. However, Basaglia is strikingly absent from the literature in Germanic and Anglophone psychiatry. Most of the literature about Basaglia in the last 40 years has been published by his followers and supporters and has often been largely positive, with little exploration of differing responses or possible limitations of his model. Basaglia's International Legacy: From Asylum to Community provides an overview of current thinking and the international influence of Franco Basaglia. This resource draws on the combined knowledge of clinicians, policy makers, historians, and social scientists, including a handful of Basaglia's collaborators. Book excerpt: Two ofthe most important notions concerning the rights of people with mental illness are among the most neglected: the first is that human rights and duties are complementary and that both must be considered in constructing a framework for mental health care. The second is that we must strive for equity and not only for equality in developing mental health programs. The first ofthese notions is complex. It refers to the duties ofpeople with mental illness and to the duties ofthose who surround them. Mental illness does not liberate the person who has it from civic obligations. The most basic ofthese is to give support to others and to refuse to harm them Their carers, society and the patient's self-respect all gain through their recognition, even though the fulfilment of these obligations might be difficult or impossible in certain periods ofillness. The duty of those surrounding the patients is to recognise and respect their existence and to make the necessary arrangements to respond to their needs, protect their rights and compensate for their temporary or permanent inability to fulfil their civic duties. A society's social capital is the public good that results from the mutual supportofmembers ofa society: iffor one reason or another, some or all members ofa society fail to offer such support the social capital will diminish and the society will cease to be civic.

This is particularly true of the mentally ill. Stereotypes are most ingrained for mental pathologies with heightened visibility in society, such as schizophrenia. Preconceived notions about danger, occult powers and mysterious malevolence which hover over the illness, contribute to the total debasement of the patient. Persons suffering from other forms of mental illness are stigmatized to a lesser degree. But the threat is real that labeling will extend to every endeavor linked to mental illness: care facilities, professionals, therapies in general and psychotropic medication in particular. Lay belief in the existence of important side-effects to this medication and public fears about the risk of addiction form the basis of very restricted, or even hostile, attitudes towards it and result in weak compliance. Inversely, psychotherapy now seems widely accepted and different forms of intervention have contributed to de-stigmatizing psychiatric illness and to stop the exclusion of patients. This book is of interest not only to psychiatrists, but also to mental health workers, psychologists, social scientists and social workers who wish to alter common precepts and prejudices regarding psychiatric disorders.
