Book excerpt: Contexte : La simulation médicale a pour objectif d'être un outil pédagogique pour la formation et l'évaluation des médecins sur des situations critiques. L'objectif de notre étude est d'évaluer les compétences et performances chez les internes d'anesthésie réanimation par l'apport de séances de simulations réalistes aux cours de deux situations critiques. Méthodes : Tous les internes de 3ème année de DES d'anesthésie-réanimation ont reçu une formation théorique au cours de leur cursus avec rappel des dernières recommandations. Deux groupes d'étudiants de 12 internes ont été formés respectivement à l'arrêt cardio-circulatoire (groupe ACC) et au choc anaphylactique (groupe CA) à T0 sur simulateur haute fidélité HPS® de CAE. A 6 semaines (T1) et à 6 mois (T2), les 2 groupes étaient évalués sur 2 scénarii : une fibrillation ventriculaire réfractaire (FV) et un choc anaphylactique de grade 3 (CA3). Les grilles d'évaluations étaient respectivement notées sur 20 et 30 points. Les comparaisons ont été réalisées à l'aide d'un test de Wilcoxon. Les données sont présentées en médiane (interquartile 25-75), p˂0.05 considéré comme significatif. Résultats : La moyenne des notes obtenues pour les scénarii CA3 et FV était significativement supérieure dans les groupes d'apprentissage respectifs à T1 (FV : 18.82 [17.35-20.28] vs 7.27 [2.89-11.66] ; CA3 : 28 [26.92-29.08] vs 18.73 [13.35-24.1]) et à T2 (FV : 18.75 [17.77-19.73] vs 13.83 [11.86-15.8] ; CA3 : 28.83 [28.13-29.54] vs 24.83 [23.08-26.59]). A 6 mois, on mettait en évidence une amélioration significative des 2 groupes pour la prise en charge des situations pour lesquelles ils n'avaient pas été formés initialement : pour le groupe ACC, scénario CA3 (18.73 [13.35-24.1] et 24.83 [23.08-26.59]) ; pour le groupe CA, scénario FV (7.27 [2.89-11.66] et 13.83 [11.86-15.8]). Conclusion : La formation pédagogique en simulation médicale améliore les performances des internes formés à une situation avec maintien des compétences à 6 mois. Une unique séance d'évaluation sur simulateur améliore également les performances d'un groupe non formé initialement. Celui-ci n'atteint pas le même niveau que ceux ayant bénéficié d'une formation pédagogique complète. L'intégration d'un programme de formation en simulation doit être à considérer pour l'apprentissage des internes d'anesthésie réanimation.

L'objectif de ce travail est de comparer l'efficacité d'un "jeu sérieux" par rapport à une simulation classique comme rappel à 3 mois d'une formation initiale à la réanimation du nouveau-né et d'évaluer l'impact sur les connaissances théoriques, les compétences techniques et non techniques et sur la confiance en eux des étudiants à prendre en charge une telle situation. La formation des étudiants à une situation de stress comme la réanimation d'un nouveau-né semble améliorée par des séances de simulation ou de jeux sérieux. Le rappel par séance de simulation tout comme un jeu sérieux permet une progression significative de la connaissance théorique du sujet et une amélioration de la confiance en soi des étudiants avec un meilleur sentiment de maitrise de la situation ainsi que des compétences non techniques comme la gestion de la situation et la communication en équipe. La simulation numérique permet donc un gain de temps et de formateur et semble efficace comme méthode pédagogique de rappel après une formation initiale.

Ceux qui exercent en anesthésie réanimation ont un risque majoré. Le secteur de la santé et la santé des individus sont au cœur du changement (Rhéaume, 2002 ; Bedin, 2013 ; Marcel, 2014). La formation des praticiens a évolué pour une approche par les compétences et intègre la simulation comme méthode de développement de ces compétences. Dans ce contexte, nous avons essayé de mettre en évidence l'impact de la simulation pleine échelle de situations critiques (Pastré, 2010) sur le stress, l'anxiété, l'épuisement professionnel et in fine sur la qualité de vie au travail des participants. Pour cela, nous avons réalisé, dans le cadre d'une démarche quasi-expérimentale, une phase exploratoire, avec 50 étudiants en soins infirmiers, pour tester trois échelles relatives à l'anxiété et au burn out. Lors de la phase 2, nous avons assisté à des séances de simulation auprès de soignants exerçant en anesthésie réanimation pendant un an et testé 209 professionnels. Ils ont renseigné les échelles avant et après la séance de simulation. Enfin, dans la phase 3, nous avons cherché à mettre en évidence l'effet de la simulation sur le stress clinique de 215 étudiants en soins infirmiers. Nous montrons les effets positifs que semble avoir la simulation sur le stress, l'épuisement professionnel, l'anxiété et les traits de personnalité anxieux des participants. D'autre part, il semblerait que la simulation puisse permettre une diminution du stress clinique des étudiants en soins infirmiers et que cet effet soit stable quatre mois après la séance de simulation. La simulation aurait ainsi un effet préventif sur le stress. Ces résultats permettent d'appuyer des propositions de développement de cette méthode pédagogique en formation initiale et continue.

Méthodes: Tous les internes de 3ième année du 3ième cycle en anesthésie-réanimation et médecine d'urgence ont été convoqués. Tous ont bénéficié d'une formation théorique avec un rappel des dernières recommandations sur la prise en charge de l'arrêt cardiaque et du choc anaphylactique. Ils ont été randomisés en 2 groupes et ont reçu une formation pratique sur simulateur haute-fidélité à la prise en charge soit de l'arrêt cardiaque (groupe ACC), soit du choc anaphylactique (groupe CA). Chaque groupe a été évalué à 6 semaines (T0), 6 mois (T1) et 1 an (T2) sur deux scénarii : une fibrillation ventriculaire réfractaire (FV) noté sur 20 points et un choc anaphylactique de grade 3 (CA3) noté sur 30 points. Chaque groupe a servi de groupe témoin à l'autre pour la pathologie dans laquelle il n'a pas reçu de formation spécifique sur simulateur. Les internes d'anesthésie-réanimation sont particulièrement susceptibles d'être réceptifs à cette méthode. Les objectifs étaient de décrire l'expérience et les attentes des internes, et d'évaluer ces caractéristiques selon leur ancienneté. Matériel et Méthodes : Il s'agissait d'une étude descriptive transversale, s'intéressant aux internes d'anesthésie-réanimation des CHU de Toulouse, Bordeaux, et Limoges. Les données ont été recueillies à l'aide d'un questionnaire informatisé de 16 questions. Les thèmes abordés étaient l'expérience du débriefing en simulation et à la suite de situations critiques en pratique clinique, les besoins et les attentes, et l'intérêt porté au débriefing en pratique réelle. L'accord parmi les participants était évalué afin de juger du caractère approprié de différentes modalités de débriefing. Les attentes des internes ont ensuite été analysées en fonction de leur ancienneté. Résultats : Un total de 93 questionnaires a été recueilli, ce qui correspondait à un taux de réponse de 34%. La majorité des internes avait au moins une expérience de débriefing en simulation (82,8%) et en pratique réelle (72%). Les taux de satisfaction respectifs étaient de 80,6% et de 60%. Il semblait exister de nombreux formats différents en pratique clinique. Le délai le plus approprié pour organiser un débriefing en pratique clinique était le lendemain de la situation critique. La population s'accordait sur l'intérêt de cette pratique. La quasi-totalité (98,9%) était favorable à sa mise en place. Les internes séniorisé montraient plus d'intérêt pour les débriefings individuels et collectifs médicaux que les juniors, et considéraient les apports psychologiques plus importants. Conclusion : Les internes en anesthésie-réanimation interrogés sont familiers et satisfaits de leurs expériences de débriefing en simulation et en pratique clinique. A partir des attentes des internes, nous avons élaboré un modèle de débriefing susceptible de convenir à la majorité.

Le savoir-faire et le savoir-être étaient les deux principaux éléments d'apprentissage. A l'issue de cette simulation, un questionnaire de satisfaction leur était remis pour évaluer leurs ressentis. Dans un second temps, nous avons comparé l'anxiété des patients après avoir été reçus en CPA par un interne ayant bénéficié ou non de la simulation. Un questionnaire était remis aux patients avant la CPA. Il évaluait le score APAIS avant et après la consultation ainsi que des éléments du savoir être de l'interne. Résultats : Le score APAIS total n'était pas significativement différent selon l'interne qui effectuait la CPA. Book excerpt: During the First European Congress of Anesthesiology, held in Vienna, Austria, in September, 1962, panel discussions on nineteen different subjects were held, each lasting approxi mately three hours. One, concerning Controversial Aspects of Resuscitation, was later edited by its chairman, PETER SAFAR, and published in 1963. At the request of the publisher, the discussion on hypnosis has been edited in a similar manner. The participants in the discussion on Hypnosis in Anes thesiology had agreed, prior to the meeting, on a list of questions to be debated, and Dr. STOKVIS' introductory statement on the nature of hypnosis was circulated among them in appropriate translations in order to give the debate a starting point. It had also been agreed that no formal papers should be read after this introduction, and that the participants should use at will the German, English or French language. Following each contribution it was the chairman's task to give a brief summary in the two other languages. Discussion was therefore somewhat slowed down and occasionally rendered difficult by misunderstandings or the omission of details. Drs. GUEGUEN, MOSCONI, and VOLGYESI, who were not able. to attend, had sent written contributions which the chairman presented briefly during the discussion. The present text has been worked out of the tape recording of the discussion and the chairman's correspondence with the panelists some time after the meeting. Whenever the original contributions were in French or German, the English translation has been the chairman's.

This volume contains Sigerist's correspondences with the architect of American medicine, William H. Welch, the pioneer brain surgeon, Harvey Cushing, the medical bibliographer, Fielding H. Garrison, and the medical historian, Erwin H. Ackerknecht. The letters allow insight into the correspondents' biographies and activities, their private lives, and relationships between persons, topics, and books. They also reflect the eventful time of the mid-20th century. To each of the four correspondences is added an introduction and indices of literary works and of persons mentioned.
