Mañana (un mañana que ya es hoy) se le añadirá todavía la responsabilidad económica, exigida por quienes gerencian los sistemas de salud. Así, cualquier clínico, además de enfrentar problemas activos de un paciente concreto, deberá enfrentar –ya lo hace hoy–problemas probabilísticos. Ayer, hoy y mañana no son sólo cambios cronológicos, sino también variaciones de pautas culturales. Sabemos que no podemos detener el tiempo; aun así, podemos defender de la erosión las pautas culturales que creemos dignas. Hasta hace algunas décadas, el médico vivía la pauta cultural de la entrega generosa y la sabiduría, que hoy debe cambiar por la de la efectividad y la eficiencia. El esfuerzo debe apuntar, entonces, a conciliar ambas culturas; el desafío es cómo hacerlo. Alberto Agrest Sus escritos, que deberían ser de lectura obligada para quienes se dedican a la medicina y más aún para las nuevas generaciones que planean hacerlo, lo han convertido ya en un clásico de la reflexión sobre el destino de la medicina contemporánea. El objeto de los capítulos III y IV se centran en el estudio del significante léxico en relación con la aparición de nuevas palabras y la desaparición de otras, así como del significado léxico, en su doble vertiente de información gramatical y contenido léxico. Distinguimos en este apartado la definición en metalengua de contenido (definición propia) de la definición en metalengua de signo (definición impropia). Un grupo importante de entradas léxicas funcionan como palabras performativas, es decir, al usarlas se convierten en actos de habla. El último capítulo está dedicado al análisis de las formas complejas. On the basis of the historical precedents and motivations that led to the concern about the language, chapter I establishes the theoretical and methodological base (Linguistic Pragmatics) for the analysis of the language of the DRAE (Diccionario de la Real Academia Española). After having established the parameters concerning the language of the dictionary, the author analyses (chapter II) the most representative articles of the academic Dictionary, which works as the center of interest because they are an expression of judgments about the society. The object of chapters III and IV is the study of the appearance of new words and the disappearance of others, as well as of the lexical meaning and grammatical information of the same ones. We do the distinction between the definition in metalengua of the content (own definition) and the definition in metalengua of the sign (the improper definition). An important group of lexical entries function as performative words, which means that when we use them, they are changed into speech acts. The last chapter is dedicated to the analysis of set phrases or locutions.

El autor esta consciente de los riesgos que puede correr un profesional de la salud mental que se destapa y desprende de todo prejuicio con la esperanza de poner a la disposicion de sus lectores un testimonio del cual pueda sustraer algo util para sus vidas y sus ocupaciones o profesiones. Habiendo enfrentado tantas situaciones de alto riesgo, el autor hace apologia del ejercito de angeles que lo han protegido y que el identifica con los seres a quien ha dado amor y respeto y que le han retribuido esos afectos en forma incondicional, tanto en su familia como en muchas personas que fue encontrando en su largo recorrido por el mundo. De formacion transcultural, el autor ha podido convivir armoniosamente con personas de diversas creencias religiosas, politicas y origen etnico. El autor ha hecho muchas renuncias excepto renunciar a vivir en libertad, lejos de los opresores de las libertades y derechos individuales, y no renuncia a la esperanza de algun dia vivir en un mundo libre de prejuicios, estereotipos y dictaduras depredadoras y brutales, ni tampoco renuncia a la esperanza de que cada ser humano pueda aprender a usar los recursos personales que todos poseemos para ser ricos, interdependientes y exitosos en vez de pobres, lastimosamente dependientes y fracasados. El mensaje que el autor intenta transmitir en sus confesiones es un mensaje de vida y prosperidad por medios eticos, repudiando enfaticamente el lema de que "los fines justifican los medios"

More or less permanent confederations were forged with the most diverse academic disciplines, and amazingly varied frameworks were devised-suited to the needs of the times and the sites-to carry the work of consolidation forward. Bit by bit, mutually supportive international networks were put together. Today, it can truly be asserted that semiotics has become a global enterprise. This, of course, is far from saying that the map is uniform or even that world-wide homogeneity is in the least desirable. While our conjoint ultimate goal remains steadily in focus, the multiplicity of avenues available for its realization is inherent in the advent ure of the search itself.
