Une étude descriptive transversale par questionnaire internet a été réalisée. Les critères d'inclusion étaient d'être végétarien, d'habiter en France, sans limites d'âge. Le questionnaire, anonyme, a été publié sur la page Facebook de l'association végétarienne de France. Le critère de jugement principal repose sur les comportements et non-dits des patients. Résultats. Entre le 27 mai et le 30 juin 2016, 1 618 réponses ont été recueillies dont 1 452 ont pu être incluses. 362 répondants (24.9%) n'ont pas dit à leur médecin qu'ils sont végétariens. Il en résulte un besoin d'écoute et de compréhension de leurs choix de vie et surtout une prise en compte de la personne dans sa globalité. Une réaction négative du médecin à l'annonce de leur végétarisme, souvent expliquée par un manque de connaissances sur le sujet, peut mettre en péril l'équilibre fragile de la relation médecin-patient.

La prévalence des végétariens augmente. 2,2 % des français déclarent avoir adopté un régime sans viande. Si réduire sa consommation de viande apparaît bénéfique pour la santé, les régimes d'éviction peuvent être à l'origine de carences et avoir un impact néfaste sur la santé. Il convient donc de s'interroger sur les pratiques des médecins généralistes face à ces patients. Méthode : Étude qualitative par entretiens individuels et semi dirigés auprès de 12 médecins généralistes, installés des Hauts de France. Les entretiens ont été menés jusque suffisance des données. Résultats : En l'absence de recommandations précises, le dépistage, le suivi clinique et le suivi biologique des patients végétariens est hétérogène. Le médecin généraliste, peu formé, apparaît en retrait face à des patients « experts », ce qui remet en question la relation hiérarchique médecin-malade. Selon les médecins, le végétarisme est avant tout un choix de vie qui se détache de l'objet sanitaire ce qui justifie leur peu d'investissement dans le suivi de ces patients. Si leurs avis divergent à propos des régimes végétariens, les médecins privilégient une posture de neutralité par respect du patient ou pour s'affranchir de leur suivi. S'agissant des enfants, des femmes enceintes et des séniors, les médecins interrogés y étaient peu confrontés et sollicitaient volontiers l'aide de spécialistes. Conclusion : Si les médecins généralistes sont conscients de leur rôle en matière de nutrition, en pratique, ces sujets sont encore trop peu abordés, ou de manière approximative. Tandis que certains évoquent un intérêt limité pour ces problématiques, la majorité des médecins sont en demande de plus de formation et d'un travail pluridisciplinaire autour de ces patients.

Matériel et méthodes : ous avons mené une étude qualitative par entretiens individuels semidirigés auprès de 17 patients végétariens. Les entretiens ont été enregistrés, retranscris puis coder à l'aide du logiciel RQDA. Résultats : Les personnes interrogées rapportaient des réactions souvent négatives de la part des professionnels de santé. ainsi que la persistante de nombreux préjugés, un manque de dialogue et la sensation que ces derniers avaient peu de connaissances concernant leur alimentation. Même si dans la plupart des cas, el les n · ont pas changé de médecin traitant, cene situation pouvait avoir des conséquences sur la qualité de la relation médecin-patient, notamment J" omission de certaines informations. Discussion : Deux autres thèses se sont intéressées aux patients végétariens et au point de vue des médecins. et elles constataient également un manque dïnformations et de compréhension de la part des professionnels de santé. Cene situation est probablement liée au fait que contrairement à d'autres pays (comme le Canada et les Etats-Unis), les institutions référentes de la santé, ne sont pas très favorables au végétarisme. Mais c'est une position qui tend à évoluer. Conclusion : Les personnes interrogées étaient globalement déçues de !"accueil et de la prise en charge de leur médecin traitant. Une plus grande tolérance et une mise à jour des connaissances sur le végétarisme pourrait permettre une amélioration des relations médecin-patient.
