La réadaptation, terme plus couramment utilisé, est l'ensemble des soins spécifique s'adressant à des malades pour lesquels une réinsertion apparaît comme problématique. Historiquement, son développement est intimement lié au phénomène de désinstitutionnalisation. Les principaux modèles s'articulent autour des théories cognitives et comportementales, et institutionnelles, avec des applications très variables d'un pays à l'autre et même d'un service à un autre. Les parcours sont également difficiles à définir et à modéliser, sur le plan théorique, et c'est à travers une étude rétrospective, initiée dans un service spécialisé de la MGEN, associant soins et réadaptation, que nous nous sommes intéressés à la place qu'occupait la réadaptation dans les parcours de soins des patients, et aux caractéristiques des patients à qui elle était proposée. Pour compléter la question de la place de la réadaptation dans les trajets de soins, nous nous sommes intéressés à trois patientes du service, qui présentent des parcours originaux : deux nous paraissaient présenter un bon pronostic, mais une seulement s'est réinsérée, et une troisième patiente semblait n'amener que peu d'espoir pour une réadaptation de bonne qualité, et sa situation actuelle nous montre l'inverse. Le choix du moment des actions de réadaptation semble être une de clés de l'efficacité de la réhabilitation psychosociale.

Ces deux composantes sont étroitement liées et sont à l’origine d’une modification au niveau psychologique sur le patient du à leur fort impact. Etant donné la position de la prothèse maxillo-faciale dans la prise en charge du patient atteint de cancer, et l’impact psychologique qui accompagne ses traitements, il semble intéressant d’évaluer la qualité de cette prise en charge tant au niveau fonctionnel que psychologique. L’élaboration d’un questionnaire et son étude à suivre devrait nous permettre d’avancer une réponse, et d’améliorer encore la qualité de cette prise en charge

Une méthodologie qualitative d'analyse thématique a été utilisée à partir de 15 entretiens semi-structurés. Les patients se représentent la réhabilitation psychosociale à partir de sa fonction, qui serait d'atteindre des objectifs définis au regard des difficultés exprimées. Ces objectifs vont être principalement de retrouver une expérience de vie positive et de changer ses activités dans et en dehors du centre. La réhabilitation psychosociale va avoir pour effet de permettre aux patients de changer leur point de vue sur eux-mêmes, de développer des capacités, de favoriser leur socialisation et leur rythme de vie et de se sentir mieux. Ces effets sont principalement attribués aux ateliers thérapeutiques. Ils sont favorisés par la motivation à poursuivre les soins, les relations bienveillantes avec les professionnels et les autres patients et le cadre facilitant l'autonomie. Le manque d'intérêt dans les prises en charge ou les conflits vont entraver les soins. Ces résultats mettent en relief l'importance d'un cadre de soin soutenant et autonomisant, qui donne la priorité aux objectifs des patients plutôt qu'à la correction des déficiences afin de favoriser le processus de soin.
