Book excerpt: L'objectif de cette thèse est d'interroger le lien existant entre la menace d'accouchement prématuré et les conflits psychiques survenant au cours de la grossesse. Dans un premier temps nous exposons les principaux concepts descriptifs de la dynamique psychique de la grossesse en considérant le monde représentationnel et fantasmatique de la femme enceinte tels qu'ils ont été décrits par les auteurs, mais aussi les différents aspects de la relation materno-foetale. Dans un deuxième temps nous évoquons le concept de la parentalité en articulant secondairement les conflits de la parentalité au concept de la transmission psychique générationnelle. Dans un troisième temps nous exposerons les travaux se portant sur le psychisme de la femme qui présentent une menace d'accoucement prématuré. Dans un quatrième temps nous rapportons trois dossiers cliniques de menace d'accouchement prématuré nous permettant d'articuler les différents concepts. Nous concluons ce travail par des hypothèses issues de la théorie et de la pratique, nous permettant de répondre, en un certain point, à notre interrogation initiale.

Son coût humain, physique, psychique et financier est important, tandis de son étiologie reste partiellement inconnue. On estime qu'environ 50% des accouchements prématurés se produisent en l'absence de facteurs de risques connus. De nombreux auteurs ont évoqué le rôle possible de facteurs psychiques, pré-existant ou non à la grossesse, à l'origine de ces accouchements prématurés sans cause connue. La menace d'accouchement prématuré (MAP), qui concerne jusqu'à 20% des grossesses, est une entité plus difficile à cerner, parfois discutée, et pour laquelle il n'existe pas, actuellement de critère de gravité fiable. L'hypothèse proposée est que les MAP sans facteur obstétrical identifiable soient l'expression somatique d'une difficulté à gérer les conflits d'ambivalence autour du désir d'enfant et des identifications maternelles, qui tiennent une place essentielle dans le travail psychique de la grossesse et de la maternité. Après un rappel des facteurs de risques médicaux et sociaux, les principales données de la littérature sur les aspects psychologiques de la prématurité sont exposés, puis les étapes de notre recherche sont présentées. Le projet consiste en la mise au point d'un auto-questionnaire spécifique, permettant de dépister les femmes à risque de menace d'accouchement prématuré d'origine psychologique, et ainsi prévenir par une prise en charge spécifique ce syndrome et ses conséquences.

Tout comme l’adolescence il s’agit d’une nouvelle étape dans la vie féminine.Les modifications du corps en lien avec la grossesse s’accompagnent de processus psychiques qui s’instaurent progressivement jusqu'à la naissance et se poursuivent en post partum.Parmi ces processus nous retrouvons les représentations mentales de l’enfant. La survenue d’un événement traumatique pendant la grossesse peut alors perturbée ces processus psychiques. C’est part le biais d’une étude qualitative portant sur neufs entretiens avec des femmes hospitalisées que nous étudions l’impact d’une menace d’accouchement prématuré sur les représentations maternelles de l’enfant mais également de la prématurité. Il apparaît ainsi que l’hospitalisation des futures mères à un impact sur les processus psychiques entrepris en début de grossesse dans le sens où l’angoisse et le stress peuvent sidérer totalement les représentations du fœtus.La prématurité et toutes les représentations qui l’entourent vont prendre place dans le psychisme des femmes. Ainsi l’enfant à venir n’est plus imaginé de la même manière qu’auparavant, il est directement associé à la prématurité.Nous remarquons également que les fantasmes concernant la prématurité sont présent chez les patientes hospitalisées et cela même si l’échographie morphologique du foetus est tout à fait normale. L’absence de support d’information sur la prématurité est clairement exprimée, en effet celui-ci pourrait sans doute aider les patientes dans l’élaboration des représentations mentales de leur enfant.
