Les femmes étant censées travailler moins que leurs confrères et privilégier leur vie familiale, nous avons voulu examiner si la féminisation de la médecine générale contribuera à une modification de l’offre de soins en Picardie. Nous avons, à l’aide de deux questionnaires, interrogé un échantillon de médecins généralistes installés en Picardie et les internes en DES de médecine générale de la faculté d’Amiens afin d’évaluer l’exercice actuel et l’exercice envisagé. 96 médecins généralistes et 175 internes en DES de médecine générale ont participé à l’étude. 59 % des internes sont des femmes. 81% des femmes déclarent vouloir exercer en Picardie contre 65% des hommes. Un interne sur deux envisage un exercice libéral. L’exercice seul au cabinet et en zone rurale sont délaissés par les femmes. Les internes envisagent de diminuer leur temps de travail et d’effectuer moins de gardes, par rapport à leurs aînés, pour préserver leur qualité de vie. La famille est une des priorités principales des jeunes médecins, qui ne souhaitent plus se consacrer exclusivement à leur exercice médical et privilégient le cabinet de groupe afin de faciliter l’organisation du temps de travail. La féminisation et le changement générationnel de la profession modifieront l’exercice de la MG. Les nouvelles priorités des futurs généralistes imposent une restructuration de l’exercice de la MG, c’est pourquoi nous proposons des aménagements basés sur les desirata des internes, c’est à ce prix que la féminisation sera un atout pour l’offre de soins en Picardie.

Book excerpt: Contexte : la médecine générale subit une crise démographique, pour laquelle les pouvoirs publics ont évoqué plusieurs explications, dont « la pénibilité de l'exercice générée par les conditions de travail et les horaires ». Cette dernière explication pourrait être d'autant plus importante à prendre en compte qu'on assiste à un phénomène de féminisation de la profession médicale : les femmes sont présumées plus enclines à rechercher un équilibre entre leur vie privée et leur exercice professionnel. La perspective d'une profession majoritairement féminine à court terme alimente alors toutes les inquiétudes concernant l'offre de soins. Objectifs : l'objectif principal est de comparer le temps et l'organisation du travail des femmes médecins généralistes, avec celui des hommes médecins généralistes, exerçant en libéral (ou mixte), sur l'ensemble du territoire français. Ce travail aspire à étudier comment le métier est investi par la génération actuelle de médecins, et dans quelle mesure la féminisation influe sur la profession. Méthode : il s'agit d'une étude quantitative observationnelle, analytique et transversale, qui se déroule sur l'ensemble du territoire français auprès des médecins généralistes thésés en exercice libéral ou mixte. Les données ont été mesurées à partir d'un auto-questionnaire, envoyé par courriel à 32 768 médecins adhérant à un organisme de formation médicale continue. L'enquête a recueilli 2 550 réponses, sur une durée d'un mois. Le critère de jugement principal était la durée de travail hebdomadaire moyenne. Résultats : le taux de participation était de 7,8 %. 61,8 % des répondants étaient des femmes. Les femmes étaient significativement plus jeunes que les hommes, exerçaient préférentiellement en zone urbaine et étaient plus souvent célibataires. La durée de travail hebdomadaire moyenne des femmes était significativement inférieure à celle des hommes. Le nombre hebdomadaire moyen de consultations réalisé par les femmes était inférieur à celui des hommes, mais leurs consultations, essentiellement sur rendez-vous, étaient significativement plus longues. La sphère familiale et parentale avait significativement plus d'influence sur la pratique des femmes comparativement aux hommes, dans les tranches d'âge les plus élevées. Conclusion : l'étude du temps de travail des médecins généralistes a révélé un temps de travail global des femmes inférieur à celui des hommes. Mais cette variable semble trop réductrice et n'explique pas à elle seule les mutations actuelles d'organisation du travail des médecins généralistes. L'étude de l'organisation du travail et l'analyse par classes d'âge a finalement révélé qu'à l'effet de genre indéniable, s'associe un effet générationnel. Les mesures à venir et celles d'ores et déjà mises en place par les autorités pour faire face à ces mutations, seront probablement à l'origine de nouvelles évolutions, démographiques et organisationnelles.
