Book excerpt: In the 1930s, the French Third Republic banned naturalized citizens from careers in law and medicine for up to ten years after they had obtained French nationality. In 1940, the Vichy regime permanently expelled all lawyers and doctors born of foreign fathers and imposed a 2 percent quota on Jews in both professions. On the basis of extensive archival research, Julie Fette shows in Exclusions that doctors and lawyers themselves, despite their claims to embody republican virtues, persuaded the French state to enact this exclusionary legislation. At the crossroads of knowledge and power, lawyers and doctors had long been dominant forces in French society: they ran hospitals and courts, doubled as university professors, held posts in parliament and government, and administered justice and public health for the nation. Their social and political influence was crucial in spreading xenophobic attitudes and rendering them more socially acceptable in France. Fette traces the origins of this professional protectionism to the late nineteenth century, when the democratization of higher education sparked efforts by doctors and lawyers to close ranks against women and the lower classes in addition to foreigners. The legislatively imposed delays on the right to practice law and medicine remained in force until the 1970s, and only in 1997 did French lawyers and doctors formally recognize their complicity in the anti-Semitic policies of the Vichy regime. Fette's book is a powerful contribution to the argument that French public opinion favored exclusionary measures in the last years of the Third Republic and during the Holocaust.

Plusieurs études évoquaient une modification des pratiques des MG femmes (diminution des horaires de travail, moindre participation à la permanence des soins). 2 thèses ont déjà étudié le ressenti des patients face à la féminisation, mais dans des départements où la féminisation des MG était supérieure à la moyenne nationale. Le taux de femmes MG est inférieur à la moyenne nationale dans le Nord. L'objectif de cette thèse est donc d'évaluer le ressenti des patients sur les répercussions potentielles de la féminisation des MG dans le département du Nord. Méthode : Il s'agit d'une étude qualitative avec entretiens semi-dirigés, réalisés à l'aide d'un guide d'entretien. Les données ont été encodées à l'aide du logiciel QRS NVivo10. Résultats : 14 patients ont été interrogés. Le genre du MG ne semblait pas intervenir dans le choix du médecin traitant. De plus, les patients ne percevaient pas de différences d'organisation entre les MG en fonction du sexe. Les femmes MG, par l'implication familiale, pouvaient être plus souvent absentes. Mais la continuité et la permanence des soins étant assurées, les patients ne ressentaient pas de répercussion sur les soins primaires. Concernant les compétences médicales, elles semblaient identiques entre les MG des 2 sexes. Quelques qualités féminines ont cependant été citées (écoute, compétences avec les enfants) mais elles ne pouvaient pas être généralisées à l'ensemble de la profession. Les patients ressentaient également une modification des pratiques, surtout due à l'évolution des MG, et non liée à la seule féminisation. Conclusion : Malgré les études évoquant l'impact de la féminisation sur les soins primaires, les patients du département du Nord ne semblaient pas en ressentir les répercussions.

Les modes d'organisation de travail du médecin généraliste connaissent des mutations, qui relèvent à la fois d'un effet de genre et d'un effet de génération. Méthode : Enquête réalisée dans le Nord Pas de Calais par des questionnaires adressés aux médecins ayant soutenu une thèse de médecine générale à Lille, en 2000 d'une part et en 2010 d'autre part. Résultats : L'exercice médical féminin repose sur la mise en place de moyens visant à maîtriser leur emploi du temps, impliquant un temps de travail inférieur aux hommes mais aussi une offre de soins plus variée ainsi qu'un niveau de satisfaction plus élevé. Il apparaît très nettement que les aspirations professionnelles et privées des femmes et des hommes tendent à s'uniformiser, avec une certaine forme de mimétisme réciproque : les hommes investissent de plus en plus le milieu salarié, et les femmes tendent à « rattraper » le niveau d'activité des hommes. Conclusion : La féminisation de la médecine générale a impulsé une nouvelle approche du métier de médecin généraliste, partagée, aujourd'hui, par l'ensemble des jeunes médecins généralistes et modifiant l'offre de soins primaires actuelle et à venir.
