Cette etude a ete realisee aupres de 40 joueurs pathologiques admis en traitement dans un centre de readaptation en dependance a Montreal. Les objectifs etaient d'evaluer: la consommation d'alcool et de drogues, la presence d'une detresse psychologique caracterisee par des symptomes et des syndromes cliniques ainsi que par des troubles de la personnalite, et la prevalence du trouble de l'ESPT dans l'echantillon. Les resultats montrent que 65 % des participants presentent une consommation problematique d'alcool; 27,5 %, une consommation problematique de drogues; 52,5 %, un diagnostic probable d'au moins un syndrome clinique (surtout anxiete et depression); 55 %, un diagnostic probable d'au moins un trouble de la personnalite; 30 %, des symptomes du trouble d'ESPT et 17,5 %, un diagnostic probable du trouble. Alors, la comorbidite est presente chez les joueurs pathologiques de l'echantillon. Il est essentiel de l'identifier pour mieux repondre aux besoins particuliers de l'individu et l'aider avec les symptomes qui aggravent le probleme de jeu et augmentent le risque de recidive."

Nous avons ensuite cherché à illustrer à l’aide de cas cliniques et d’un travail expérimental l’influence des comorbidités psychiatriques sur la prise en charge des joueurs pathologiques en demande de traitement. Afin de déterminer les comorbidités psychiatriques présentes chez les joueurs pathologiques en demande de traitement, 22 patients ont passé 4 questionnaires : le MINI, le TCI, le test de Fagerström et la SOGS. Nos résultats montrent que les joueurs pathologiques en demande de traitement présentent de très nombreuses comorbidités psychiatriques, et des dimensions tempéramentales et caractérielles spécifiques. Ces résultats suggèrent la nécessité d’une double prise en charge psychiatrique et psychologique chez ce type de patients afin d’améliorer l’efficacité du traitement.

Book excerpt: Le jeu pathologique, médicalement défini par le DSM-III depuis 1980 parmi les troubles du contrôle des impulsions, est aujourd'hui reconnu comme une addiction comportementale à part entière. Cette pathologie n'a pas encore été étudiée au plan épidémiologique en France, mais concernerait un nombre de personnes croissant avec le développement de l'offre de jeux d'argent et de hasard. Les principaux acteurs de cette offre sont les casinos, les sociétés de courses de chevaux (Pari Mutuel Urbain) et la Française des Jeux, cette dernière institution faisant l'objet d'une observation récemment mise en place par les pouvoirs publics afin d'encadrer les pratiques de jeu et de favoriser un jeu "responsable ". L'objet de ce travail, où est présentée une enquête réalisée en population toxicomane, a été d'évaluer la présence de joueurs pathologiques parmi des patients dont l'histoire addictive a été explorée au moyen d'instruments standardisés (South Oaks Gambling Screen, CAGE, Test de Fagerstrom, Severity of Dependence Scale) et de questions ouvertes. Dans un échantillon de trente-trois toxicomanes, la rencontre de huit joueurs pathologiques (24%) confirme l'intérêt d'un meilleur dépistage de cette pathologie, ainsi que d'études en population générale. Les liens de comorbidité entre toxicomanie et jeu pathologique ont été plus particulièrement étudiés selon les trois axes psychopathologiques que sont le rapport à la figure paternelle et à la Loi d'une part, une appétence particulière pour les sensations fortes d'autre part, enfin, une relation au risque de type ordalique. Les limites de cette étude résident actuellement dans l'absence de groupe contrôle où pourraient être étudiées les comorbidités addictives chez des joueurs pathologiques

Les principales comorbidités retrouvées dans le jeu pathologique sont : les troubles de l'humeur (épisodes dépressifs majeurs chez 31 à 75 % des joueurs pathologiques selon les études ; et troubles bipolaires : la manie/l’hypomanie sont représentés dans des taux de 17 à 25%) et les troubles addictifs (alcool : 28 à 73 % des joueurs pathologiques, et tabac : de 60 à 80 % ; selon les études). L’évaluation et la mise en place d’un projet de soins personnalisé est une étape importante de la prise en charge des joueurs pathologique. L'évaluation initiale d'un joueur pathologique s'effectue sur plusieurs axes: addictologique (recherche de co-addiction), psychiatrique (recherche de comorbidités), psychologique (évaluation du stade motivationnel, des cognitions erronées en situation de jeu et prévention des rechutes), et sociale. La prévalence importante des comorbidités psychiatriques chez les joueurs pathologiques en fait un élément central de la prise en charge.

It is hypothesized that tools such as games, virtual reality, or applications for smartphones may foster learning, enhance motivation, promote behavioral change, support psychotherapy, favor empowerment, and improve some cognitive functions. Computers and games may create supports for training or help people with cognitive, emotional, or behavioral change. Games take various formats, from board games to informatics to games with interactive rules of play. Similarly, computer tools may vary widely in format, from self-help or assisted computerized training to virtual reality or applications for smartphones. Some tools that may be helpful for mental health were specifically designed for that goal, whereas others were not. Gamification of computer-related products and games with a numeric format tend to reduce the gap between games and computers tools and increase the conceptual synergy in such fields. Games and computer design share an opportunity for creativity and innovation to help create, specifically design, and assess preventive or therapeutic tools. Computers and games share a design conception that allows innovative approaches to overcome barriers of the real world by creating their own rules. Yet, despite the potential interest in such tools to improve treatment of mental disorders and to help prevent them, the field remains understudied and information is under-disseminated in clinical practice. Some studies have shown, however, that there is potential interest and acceptability of tools that support various vehicles, rationales, objectives, and formats. These tools include traditional games (e.g., chess games), popular electronic games, board games, computer-based interventions specifically designed for psychotherapy or cognitive training, virtual reality, apps for smartphones, and so forth. Computers and games may offer a true opportunity to develop, assess, and disseminate new prevention and treatment tools for mental health and well-being. Currently, there is a strong need for state-of-the-art information to answer questions such as the following: Why develop such tools for mental health and well-being? What are the potential additions to traditional treatments? What are the best strategies or formats to improve the possible impact of these tools? Are such tools useful as a first treatment step? What is the potential of a hybrid model of care that combines traditional approaches with games and/or computers as tools? What games and applications have already been designed and studied? What is the evidence from previous studies? How can such tools be successfully designed for mental health and well-being? What is rewarding or attractive for patients in using such treatments? What are the worldwide developments in the field? Are some protocols under development? What are the barriers and challenges related to such developments? How can these tools be assessed, and how can the way that they work, and for whom, be measured? Are the potential benefits of such products specific, or can these additions be attributed to nonspecific factors? What are the users’ views on such tools? What are the possible links between such tools and social networks? Is there a gap between evidence-based results and market development? Are there any quality challenges?
