Available in PDF, EPUB and Kindle. Book excerpt: Marc A. Rodwin draws on his own experience as a health lawyer--and his research in health ethics, law, and policy--to reveal how financial conflicts of interest can and do negatively affect the quality of patient care. He shows that the problem has become worse over the last century and provides many actual examples of how doctors' decisions are influenced by financial considerations. We learn how two California physicians, for example, resumed referrals to Pasadena General Hospital only after the hospital started paying $70 per patient (their referrals grew from 14 in one month to 82 in the next). As Rodwin writes, incentives such as this can inhibit a doctor from taking action when a hospital fails to provide proper service, and may also lead to the unnecessary hospitalization of patients. We also learn of a Wyeth-Ayerst Labs promotion in which physicians who started patients on INDERAL (a drug for high blood pressure, angina, and migraines) received 1000 mileage points on American Airlines for each patient (studies show that promotions such as this have a direct effect on a doctor's choice of drug). Rodwin reveals why the medical community has failed to regulate conflicts of interest: peer review has little authority, state licensing boards are usually ignorant of abuses, and the AMA code of ethics has historically been recommended rather than required. He examines what can be learned from the way society has coped with the conflicts of interest of other professionals --lawyers, government officials, and businessmen--all of which are held to higher standards of accountability than doctors. And he recommends that efforts be made to prohibit and regulate certain kinds of activity (such as kickbacks and self-referrals), to monitor and regulate conduct, and to provide penalties for improper conduct. Our failure to face physicians' conflicts of interest has distorted the way medicine is practiced, compromised the loyalty of doctors to patients, and harmed society, the integrity of the medical profession, and patients. For those concerned with the quality of health care or medical ethics, Medicine, Money and Morals is a provocative look into the current health care crisis and a powerful prescription for change.

And, the numbers are expected to increase--by the year 2020, nearly 30 million Americans will have cataracts. He discovered the natural principles, normal function of the eyes (visual system) and applied natural methods, relaxation to return the eyes, eye muscles, nerves, mind/brain, body (entire visual system) to normal function with healthy eyes and clear vision. He cured; unclear close and distant vision, astigmatism, crossed, wandering eyes, cataracts, glaucoma, and other conditions. Natural Eyesight Improvement was practiced years before Dr. Bates discovered it. It is the normal, natural function of the eyes. Hidden from the public by eye surgeons, Optometrists, Optical businesses for over 100 years because this method works, is easy, anyone can learn, teach it, including children. It produces healthy eyes, clear vision and frees the patient from the need to purchase eyeglasses, drugs, unnecessary eye surgery. Yes, it can and has reversed cataracts! Dr. Bates worked his entire life treating people successfully with Natural Eyesight Improvement. When he cured the eyes, vision of many patients, medical students and other doctors in the hospital where he worked with natural treatments, without use of eyeglasses, surgery, drugs and proved his method is fact, that some of the old theories of eye function are incorrect; the eye surgeons that preferred to sell eyeglasses, surgery, drugs became angry and expelled him. (See: `Reason and Authority' & `Dr. Bates Lecture' in Better Eyesight Magazine: November, 1919, April, 1923 & Articles in his book.) Dr. Bates then opened his own office, a Clinic in Harlem, New York City. He treated thousands of people by natural methods, including many of the poor people that had little money. He kept his price for medical treatment low and also provided no charge office visits `Free Clinic Days' for people that could not afford to pay for a visit to an Ophthalmologist. His treatments were successful. He cured the young and old, people of all ages, nationalities, cured a variety of eye conditions. The Bates Method is so simple and effective that many of his cured patients, `often children' then went on to cure their friends, family, parents, teachers and other children of defective vision including crossed, wandering eyes. Read the `true story of the two little girls that restored a blind mans eyesight' in the Oct. 1925 Magazine Issue. Dr. Bates recorded 11 years of work in his clinic, his patients and their varied treatments in his Better Eyesight Magazines, Books and Medical Articles. A wide range of settings are considered, including cataract surgery in patients with uveitis, traumatic cataract, dislocation of an intraocular lens, intumescent cataract, brunescent cataract, combined cataract and corneal disease, combined cataract and glaucoma, pediatric cataract, and refractive cataract surgery. In addition, preoperative factors conducive to a favorable surgical result are identified and appropriate management of postoperative complications, described. As longevity increases, cataract is becoming an even greater public health issue worldwide. The use of cataract surgery, already one of the most frequently performed surgeries, is consequently increasing still further – a trend reinforced by developments in instrumentation and technology and improvements in access to surgery. However, comorbidities and challenges during surgery are very common. Challenges in Cataract Surgery will be a valuable reference for all residents, fellows, and practicing ophthalmologists who wish to improve their surgical techniques and outcomes.

Book excerpt: A concise review and comprehensive guide to improving cataract surgery outcomes Significant advances have been made in refractive surgery in the last decade. Eye surgeons today can leverage many cutting-edge intraocular lens (IOL) technologies such as multifocal, extended depth of field, toric, accommodating, and aspheric. Concurrent innovations including optics, phacoemulsification, superior keratometry and biometry, and posterior cornea evaluation have resulted in improved cataract surgery outcomes. Recent advances in the diagnosis and treatment of cataracts have dramatically improved patient satisfaction following cataract surgery, yet common and rare complications still occur such as dry eye, endophthalmitis, and retinal detachment. Furthermore, only 80% of cataract surgery procedures bring patients to within 0.5 diopters of their needed refraction. Optimizing Suboptimal Results Following Cataract Surgery: Refractive and Non-Refractive Management presents the latest techniques for achieving optimal results and overcoming complications in cataract surgery. Internationally renowned ophthalmic surgery pioneers Priya Narang and William Trattler have created an unparalleled guide to overcoming complex cases and residual refractive errors. The book starts with six chapters on suboptimal causes and considerations, followed by dedicated sections covering refractive and non-refractive enhancements. The fourth section features chapters on keratoconus, cystoid macular edema, endophthalmitis, toxic anterior segment syndrome, and vitritis, as well as intraoperative aberrometry, futuristic approaches and advancements, and telescopic IOLs. Key Highlights Refractive enhancement procedures including corneal-based procedures such as astigmatic keratotomy, LASIK, PRK, and SMILE; IOL exchange; piggyback IOL; toric IOL; and premium IOLs and associated problems Nonrefractive enhancement procedures including dysphotopsias and surgical management, bullous keratopathy and endothelial keratoplasty, complex IOL issues such as malpositioned IOL and capsular bag, and posterior capsular rupture and IOL lens implantation Practical tips and steps to prevent and manage undesired outcomes 30 surgical videos posted online highlight a diverse array of complex cases and technical advances to help prevent suboptimal results Ophthalmologists, ophthalmology residents, fellows, and subspecialists will greatly benefit from the significant pearls and knowledge presented in this indispensable cataract surgery resource.
