Example Chapter Threading the catheter At this point both the tip of the needle and the tip of the catheter are inside the vein. You will now need to thread the rest of the catheter into the vein. The catheter is fully inside the vein when only the hub sticks out. There are several different methods to advancing the catheter The Two Handed Technique 1.You have already visualized flash, lowered your angle, advanced 1-2mm and released the tourniquet. 2.Continue holding the needle in place 3.With your non-dominant hand, grasp the hub of the catheter and slide it forward into the vein 4.Slowly withdraw the needle 5.Place your thumb on the hub of the catheter to hold it in place 6.Use your index finger to press down above the site to prevent blood from spilling out The Single Handed Technique 1.You have already visualized flash, lowered your angle, advanced 1-2mm and released the tourniquet. 2.Continue holding the needle in place. 3.Use the index finger of the hand that is holding the needle to slide the catheter past the needle 4.Continue sliding the catheter until it is fully inside the vein. With the above two techniques, be careful that you are only advancing the catheter into the vein and not the needle. If the catheter is inside the vein it should be relatively easy and painless to advance it. Unlike the needle, the catheter is soft and flexible so it will be difficult to slide it in unless you are inside the vein. If the flashback was very small, you may feel resistance... Adult patients often state that this procedure causes considerable discomfort. Local anesthesia for cannulation is usually not offered to adult patients who are on general medical units. Evidence has indicated inconsistency in the use of pain management strategies during these procedures. The researcher’s goal in this project was to educate RNs about the intradermal pretreatment procedure, provide education on the hospital’s current IV therapy pretreatment policy, increase the usage of intradermal local anesthesia for cannulation for patients’ comfort level, and utilize low-fidelity simulation for adult learners. Many studies have shown simulation can be an influential tool in helping adult learners; for example, RNs can envision concepts and apply knowledge in a safe, nonthreatening environment. The conceptual framework the researcher chose for this simulation, point of interest capstone project was Knowles’s adult learning theory, which describes methods to help adult learners visualize the connectedness of concepts and applications of knowledge to actual life situations.

The use of a blended online learning module and live simulation has been found to increase peripheral IV insertion confidence and competence (Schuster Stahl, Murray, & Glover, 2016; Schuster, Stahl, Murray, Keleekai & Glover, 2016). Internal application of a blended curriculum model for nurses indicates improved first attempt IV insertion success. Three online modules focusing on the standards of care of peripheral IV insertion, infection prevention and documentation, as well as phlebitis assessment for IVs previously inserted were completed followed by a simulation IV competence session. Student volunteers served as the patients with a practice forearm attached to the arm for cannulation. This live interaction aided in simulating a real IV insertion and assessment experience and required the learners to interact with the patient in describing the procedure. The Peripheral Intravenous Cather Insertion Confidence Assessment (validated tool) was completed by participants (n=38) pre intervention, immediately post intervention and 30 days post intervention. A Mann-Whitney U test was completed and determined that nurses’ confidence was significantly higher immediately following the intervention compared to pre intervention (p=0.003) and the increase was sustained 30 days after the intervention (p
