Book excerpt: When antibiotics became readily available in the 1950s, the danger of life-threatening infectious childhood diseases virtually disappeared. In that era, pediatricians broadened the core professional task of their specialty--the prevention and treatment of such diseases--to incorporate the behavioral and psychosocial problems of children and adolescents. Pediatricians themselves began to refer to this changing emphasis as the "new pediatrics," and to see the trend as a natural progression of their specialty into new areas of care. At the same time there arose widespread disaffection among practicing general pediatricians, defection to other areas of practice, and a decline in the popularity of pediatrics as a specialty choice. In analyzing the emergence of the new pediatrics as a case study within medical sociology, Pawluch shows how professional concerns and interests infl uence debate around social problems. As sociologists began to take greater interest in the problems of childhood, and as children's lives became increasingly medicalized--as some have argued--it is at least in part because of pediatricians' willingness to endorse medical defi nitions for certain social problems and to provide treatment for them.Pawluch's underlying concern is that medical professionals have begun to make claims for authority in the definition of what constitutes the social problems of childhood. Among the topics she examines are the "dissatisfied pediatrician syndrome," the potential for a crisis in oversupply of pediatricians and competing providers of services, the push for expansion into new areas of care, and possible future developments in this specialty.

Moon brings her considerable expertise to the topic of Everyday Ethics in the Clinical Practice of Pediatrics and Young Adult Medicine. Conflicting moral obligations abound in pediatric medicine; competence in identifying and managing ethical concerns is integral to competence in pediatrics. Because most of pediatric care occurs in the outpatient setting, ethical issues arise often in routine clinical practice. This first-ever issue is devoted to medical ethics for the pediatrician, keeping in mind the rights and obligations of the physician, patient, and parent, as well as an understanding of widely accepted principles of medical ethics. Contains 11 practice-oriented topics including practical ethics for pediatricians; shared decision making in pediatrics; pediatrician as advocate and protector: balancing interests of children vs parents; collaborating with families on goals of care in a setting of medical complexity; and more. Provides in-depth clinical reviews on everyday ethics in the clinical practice of pediatrics and young adult medicine, offering actionable insights for clinical practice. Presents the latest information on this timely, focused topic under the leadership of experienced editors in the field. Authors synthesize and distill the latest research and practice guidelines to create clinically significant, topic-based reviews.

Book excerpt: This unique title is designed to illustrate and foster how a closer working relationship between pediatricians and subspecialists can make childhood medicine work more seamlessly. Despite the common lack of training for pediatricians in pediatric neurosurgery, they are challenged almost daily with caring for children with neurologic conditions. Common Neurological Conditions in the Pediatric Practice is replete with a wide range of instructional case vignettes and is organized into sections that loosely approximate the neurologic development of a child and address issues that are commonly encountered. The first section reviews neurologic development and birth related trauma commonly seen in the neonatal intensive care unit. The second part addresses findings commonly encountered by a pediatrician in a child’s first month of life. The third section is a comprehensive review of hydrocephalus. Part four describes state of the art imaging techniques for the central nervous system in children, from pre-natal ultrasound through MRI and CT; and the fifth part consists of individual explorations of common neurosurgical conditions that many pediatricians are uncomfortable managing, including brain tumors, spasticity, and vascular lesions to use as a reference tool when caring for a complex neurosurgical patient. Finally a series of chapters related to head trauma, including sections on non-accidental trauma and concussion management, completes the text.

Available in PDF, EPUB and Kindle. Book excerpt: This book is addressed to all professionals concerned with the health care of children. It is, first and foremost, a teaching tool. It can be used for class discussion or case conferences with medical students or residents, nurses, and other staff in pediatrics or family medicine. It can also be used for self-teaching or continuing education by those already in practice. No one who reads this book is a beginner at moral reasoning. However, many may well be beginners at discussions that focus sharply on the ethical issues in medicine and introduce philosophical analysis. The goal is to clarify, conceptualize, and guide reasoning in order to come to conclusions that can be defended with good reasons. Case studies provide the most successful method of teaching medical ethics, posing the issues as they arise in real-life situations. The cases in this book are brief and rather skeletal in nature. This is partly to deflect the natural curiosity of those who are driven to seek more and more medical details, hoping thus to resolve the ethical issues or avoid them entirely. It also allows the reader to concentrate on one ethical issue at a time. In real life, of course, the hard questions arise often several at a time, embedded in a rich and complex medical and psychosocial background. As one must learn to walk before one can run, so it is wise to practice on cases where the key issue is highlighted.
