Available in PDF, EPUB and Kindle. Book excerpt: Surgical resection of lung metastases is a widely accepted procedure but long-term results are disappointing with a 5-year survival rate of approximately 40%. The main prognostic factor is complete resection and a better survival is reported in patients with a single metastasis and a disease-free survival of more than 3 years. Re-operations are feasible but often patients become inoperable due to insufficient pulmonary reserve and new treatment modalities are looked for. Intravenous (iv) chemotherapy has no major impact on survival due to systemic side-effects when high doses are given. As isolated limb and liver perfusion, isolated lung perfusion (ILuP) has the advantage of selectively delivering an agent into the lung while diverting the venous effluent. This allows the drug to be given in a significantly higher dose compared to iv therapy, while drug levels in the other organs are kept low enough to avoid major complications. ILuP has been shown to be very effective for the treatment of lung metastases in animal models. Phase I clinical studies to determine dose-limiting toxicity and maximum tolerated dose have recently been reported. The principal aim of this book is to discuss the current treatment of lung metastases and the possible contribution of ILuP.

Book Synopsis Locoregional Tumor Therapy by : Eric Van Cutsem
Download or read book Locoregional Tumor Therapy written by Eric Van Cutsem and published by Springer. This book was released on 2018-03-09 with total page 272 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book presents, in condensed form, the clinical results achieved by means of various locoregional tumor therapies employed for different indications. In so doing, it will allow the reader to rapidly retrieve the information on indications and effectiveness that is required for optimal integration of these highly effective therapies into modern, complex treatment strategies. The renaissance in locoregional tumor therapies is an ongoing process. New chemotherapeutic schedules, especially for liver and lung tumors, have been developed during the past few years. Other targeted therapies, such as radioembolization, microwave ablation, and HIFU techniques, and new pharmaceuticals for intra-arterial therapy have been developed and implemented in clinical practice. Furthermore, the advantages of unlimited combination of starch microspheres with various antitumor agents, supported by effective ischemia, have been increasingly exploited. The second edition of this book reflects these advances and includes entirely new chapters on the role of intraperitoneal perfusion in treating peritoneal carcinomatosis and the use of limb perfusion. Locoregional Tumor Therapy will be invaluable for all practitioners who work with cancer patients.

Book Synopsis Extreme Hepatic Surgery and Other Strategies by : Eduardo de Santibañes
Download or read book Extreme Hepatic Surgery and Other Strategies written by Eduardo de Santibañes and published by Springer. This book was released on 2017-07-26 with total page 383 pages. Available in PDF, EPUB and Kindle. Book excerpt: This book describes the diagnoses; staging and management of patients with colorectal liver metastases initially considered unresectable and portrays the different strategies to increase resectability along with their tactics and tricks. Colorectal carcinoma is the third most commonly diagnosed cancer in the world and according to recent cancer statistics around 1.23 million patients are diagnosed each year. Of these patients, approximately 50% will develop liver metastases during the course of their disease and around 15-25% are found to have stage IV disease at diagnosis. Liver resection has been recognized as the treatment of choice for these patients, offering overall 5-year survival rates of up to 50-60% and the only hope for cure. However, at diagnosis only 10-20% of these patients are possibly amenable to surgical resection with curative intent. The possibility to achieve an R0 resection is many times limited by the amount and quality of the future liver remnant (FLR), being posthepatectomy liver failure (PHLF) the most feared and severe complication after major liver resections. With the years, diverse strategies have been developed with the intention to increase resectability by increasing the future liver remnant and/or reducing tumor size, e.g. ALPPS. Along with these techniques, associated surgeries are developed including multivisceral resections, which broadens even more the resectability for patients.
