Book excerpt: This book presents an innovative approach to clinical assessment in psychiatry based on a number of psychopathological dimensions with a presumed underlying pathophysiology, that are related to fundamental phenomenological aspects and lie on a continuum from normality to pathology. It is described how the evaluation of these dimensions with a specific, validated rapid assessment instrument could easily integrate and enrich the classical diagnostic DSM-5 or ICD-10 assessment. The supplemental use of this dimensional approach can better capture the complexity underlying current categories of mental illness. The findings from a large patient sample suggest how this assessment could give a first glance at how variable and multifaceted the psychopathological components within a single diagnostic category can be, and thereby optimise diagnosis and treatment choices. Being short and easy to complete, this dimensional assessment can be done in a busy clinical setting, during an ordinary psychiatric visit, and in an acute clinical context, with limited effort by a minimally trained clinician. Therefore, it provides interesting and useful information without additional costs, and allows research work to be performed even in difficult settings.

Major debate arose within the DSM process about the criteria for changing criteria, leading to the creation of first the Scientific Review Committee and then a series of other oversight committees which weighed in on the final debates on the most controversial proposed additions to DSM-5, providing important influences on the final decisions. Contained within these debates were a range of conceptual and philosophical issues. Some of these - such as the definition of mental disorder or the problems of psychiatric “epidemics” - have been with the field for a long time. Others - the concept of epistemic iteration as a framework for the introduction of nosologic change - are quite new. This book reviews issues within psychiatric nosology from clinical, historical and particularly philosophical perspectives. The book brings together a range of distinguished authors - including major psychiatric researchers, clinicians, historians and especially nosologists - including several leaders of the DSM-5 effort and the DSM Steering Committee. It also includes contributions from psychologists with a special interest in psychiatric nosology and philosophers with a wide range of orientations. The book is organized into four major sections: The first explores the nature of psychiatric illness and the way in which it is defined, including clinical and psychometric perspectives. The second section examines problems in the reification of psychiatric diagnostic criteria, the problem of psychiatric epidemics, and the nature and definition of individual symptoms. The third section explores the concept of epistemic iteration as a possible governing conceptual framework for the revision efforts for official psychiatric nosologies such as DSM and ICD and the problems of validation of psychiatric diagnoses. The book ends by exploring how we might move from the descriptive to the etiologic in psychiatric diagnoses, the nature of progress in psychiatric research, and the possible benefits of moving to a living document (or continuous improvement) model for psychiatric nosologic systems. Book excerpt: Reading philosophy through the lens of Alfred Hitchcock’s Vertigo, Andrea Cavalletti shows why, for two centuries, major philosophers have come to think of vertigo as intrinsically part of philosophy itself. Fear of the void, terror of heights: everyone knows what acrophobia is, and many suffer from it. Before Freud, the so-called “sciences of the mind” reserved a place of honor for vertigo in the domain of mental pathologies. The fear of falling—which is also the fear of giving in to the temptation to let oneself fall—has long been understood as a destabilizing yet intoxicating element without which consciousness itself was inconceivable. Some went so far as to induce it in patients through frightening rotational therapies. In a less cruel but no less radical way, vertigo also staked its claim in philosophy. If Montaigne and Pascal could still consider it a perturbation of reason and a trick of the imagination which had to be subdued, subsequent thinkers stopped considering it an occasional imaginative instability to be overcome. It came, rather, to be seen as intrinsic to reason, such that identity manifests itself as tottering, kinetic, opaque and, indeed, vertiginous. Andrea Cavalletti’s stunning book sets this critique of stable consciousness beside one of Hitchcock’s most famous thrillers, a drama of identity and its abysses. Hitchcock’s brilliant combination of a dolly and a zoom to recreate the effect of falling describes that double movement of “pushing away and bringing closer” which is the habitual condition of the subject and of intersubjectivity. To reach myself, I must see myself from the bottom of the abyss, with the eyes of another. Only then does my “here” flee down there and, from there, attract me.
